Worksite: Leech Lake Diabetes Center/Outreach Clinics
Work Hours: 8:00am to 4:30pm, 3-5 days/week, occasional evenings and weekends

License/Certification Requirements: Valid Registered Dietitian license from the State of Minnesota (MN). CPR/BLS certification/maintenance. Valid MN driver's license, current insurance or be insurable.
Summary: Provide medical nutrition therapy (MNT) and diabetes self-management education (DSME). Promote awareness of healthy nutrition practices to support the prevention and management of nutrition-related chronic diseases, with a strong emphasis on prediabetes and diabetes.
Education Requirements and Experience:
Bachelor's degree in Dietetics or a related science field, i.e. Nutrition Science.
Must be a Registered Dietitian licensed in the state of Minnesota with current licensure.
Certified Diabetes Care and Education Specialist (CDCES) preferred or willingness to obtain within a specified timeframe.
Additional Qualifications and Skills:
All licensing renewal requirements as specified by the MN State Board of Dietetics and Nutrition Practice, including required CEU's.
Excellent time management, organization, conflict resolution, public relations and speaking skills, and meticulous attention to detail.
Ability to establish and maintain an effective working relationship with staff, clients, and other professionals.
Ability to effectively and efficiently communicate verbally and in writing.
Good understanding of diabetes diagnoses, pathophysiology, treatments, health promotion, and disease prevention.
Experience and knowledge of diabetes patient education principals and methods.
Experience in utilization of Electronic Health Record (EHR) technology.
Native American preference.
Experience working with Native American clients and communities.
Familiarity with the Leech Lake Reservation and the surrounding communities is preferred.
Must meet Health Division hiring guidelines.
Pre-employment drug testing is mandatory for all Leech Lake Band of Ojibwe (LLBO) safety sensitive positions.

Duties/Responsibilities:
Functions within the scope and standards of licensure while demonstrating competency.
Functions within the policies and procedures of LLBO, Leech Lake Health Division, Indian Health Services, granting agency(s), and other organizations as appropriate.
Will provide MNT and DSME to clients, their families, communities, and health professionals in multiple settings; i.e. Tribal clinics, community centers, schools, and other sites as needed.
Will utilize the most current American Diabetes Association Standards of Care in Diabetes and/or other assigned curriculum to guide diabetes-related management and education.
Will utilize educational methods and principles to provide diabetes-related education.

Duties/Responsibilities (continued):
Will perform appropriate, accurate and complete medical record documentation.
Will assist in maintaining the most current and evidence-based educational material for use with clients and healthcare professionals.
Collaborates with health professionals to plan, implement and evaluate patient care/patient plan of treatment.
Collaborates with other community health resources to support coordinated diabetes prevention and management efforts through community-based education, outreach activities, and group classes.
Assist with program planning, development, implementation, monitoring, and evaluation to support initiatives.
Assist with grant requirements, including: chart reviews, data collection, reporting and as assigned.
Assist in quality improvement methods.
Attend and/or conduct all mandatory meetings and trainings as required.
Ability to travel in and out of state as needed.
Maintain confidentiality and professional ethics at all times.
Understanding and sensitivity to Native American culture
Adhere to policies governing staff conduct
All other related duties as assigned.
