As the Nurse Manager of Case Management leading this service in a 600+ bed hospital, community-focused care setting, the role champions evidence-based practice, social determinants navigation, and throughput optimization while partnering closely with physicians, nursing, social work, pharmacy, rehabilitative services, and payers to reduce readmissions and length of stay without compromising what matters most—outcomes and experience for every patient and family.

Reporting to a Director of Case Management or an executive over Population Health, the Nurse Manager of Case Management will have operational and leadership oversight for RN Case Manager teams, social workers, and support staff, with access to physician advisors, data analysts, clinical documentation specialists, and a dedicated educator to enhance competencies and compliance; the RN Case Manager leader will drive daily huddles, round with interdisciplinary partners, collaborate on complex discharges, steward regulatory readiness, strengthen payer partnerships, refine order sets and protocols, and coach a friendly, cohesive, and diverse team of nurses and allied professionals who value respect, empathy, and shared wins.
