POSITION SUMMARY:
The Resident Care Coordinator is responsible for providing direct nursing care to the residents and supervising the day-to-day nursing activities performed by residential aides. The nurse ensures compliance with federal, state, and facility regulations, including NH 804 and 805 regulations, to maintain the highest standard of quality care at all times. The Resident Care Coordinator holds administrative authority and accountability necessary for fulfilling assigned duties. The Resident Care Coordinator will be responsible for establishing and implementing wellness services for Independent Living Residents.
MAJOR DUTIES AND RESPONSIBILITIES:

· Direct and oversee resident care, ensuring adherence to facility regulations and policies, in coordination with the Director of Assisted Living and Memory Care.
· Direct and oversee Residential Aides, ensuring adherence to facility regulations and policies, in coordination with the Director of Assisted Living and Memory Care.
· Participate in the on-call rotation schedule.
· Participate in surveys and inspections conducted by authorized government agencies.
· Maintain a current Resident Care/Wellness Coordinator Procedure Manual that reflects day-to-day nursing procedures.
· Prepare written and oral reports/recommendations regarding shift activities as required.
· Complete all necessary reports including incident reports, medication error forms and workers compensation, in compliance with policies and procedures.
· Ensure proper documentation for admissions and discharges in resident medical records.
· Chart descriptive and informative nurses' notes reflecting resident care and response.
· Record and transcribe physician orders accurately and timely.
· Review medication orders for completeness and accuracy. Ensure residents receive an annual review of all medication orders, per regulations.
· Review and update resident care plans, including the nursing services portion.
· Provide accurate and timely nursing reports to the Director of Assisted Living and Memory Care.
· Report complaints and grievances to the Director of Assisted Living and Memory Care.
· Enforce the Five Rights of Medication Administration. This includes training Residential Aides, utilizing the 4-Hour Medication Management Training, ongoing in-service trainings, and annual review with each Residential Aide.
· Monitor medication and supply needs.
· Order medications, supplies, and equipment following established policies.
· Enforce accurate narcotic records for the shift.
· Notify the Director of Assisted Living and Memory Care of any drug and narcotic discrepancies identified.
· Coordinate with Providers to ensure comprehensive care for residents.
· Assist in the care planning process, including writing care plans, reviewing them with residents and/or resident representatives and ensuring Residential Aides follow care plans.
· Regularly check on prescribed treatments' proper administration and residents' physical and emotional status.
· Complete nursing services as needed. Must be an RN or LPN.
· Monitor residents routinely for illness and changes to physical or cognitive status.
· Regularly meet residents and/or family members to identify and address problem areas.
· Ensure resident care is provided with kindness, dignity, and respect, maintaining residents' privacy at all times.
· Honor resident's refusal of treatment requests in line with facility policies on advance directives.
· Facilitate an effective onboarding training program for new employees.
· Educate and train employees regarding best care practices protecting the resident and the employee.
· Monitor compliance with safety regulations and policies.
· Ensure the safe operation of all equipment and supplies, provide training for equipment use as needed.
· Ensure an adequate supply of personal protective equipment is readily available.
· Ensure work areas and resident rooms are clean and free of hazards.
· Report hazardous conditions or equipment and ensure compliance with safety protocols.
· Participate in emergency and disaster preparedness drills, ensuring Residential Aides and residents follow facility regulations and policies.
· Enforce infection prevention and control policies and practices.
· Provide data to the Quality Assurance committee, and participate, as requested.
· Participate in all other assigned committee, as appropriate.
Projects a positive image of the organization to employees, residents, visitors and outside community.
Other duties as assigned by the Director of Assisted Living and Memory Care.
QUALIFICATIONS:
· Active and unencumbered NH Nursing License (LPN or RN)
· Two years’ experience working in healthcare setting, preferably long-term care, or related field.

WORKING CONDITIONS/PHYSICAL ENVIRONMENT:
· Works in resident service areas as well as an office area.
· Sits, stands, bends, lifts and move intermittently during working hours.
· Is subject to frequent interruptions.
· Maintains flexibility to address any resident care or staffing issues that arise outside of business hours, via an on-call rotation.
· Is subject to exposure to infectious waste, diseases, conditions, etc., including but not limited to COVID-19, and influenza viruses. Personal protective equipment is available and must be used according to facility regulations and policies.
· Compliance with facility regulations and policies related to the work environment, including health and safety standards, must be adhered to.
Pay: From $65,000.00 per year
