Full Job Description

Registered Nurse (RN) – Clinical Care Coordinator

Avenix Health and Wellness
Voorhees, NJ

Position Summary

Avenix Health and Wellness is seeking a compassionate, professional and clinically skilled Registered Nurse (RN) to provide patient care in our outpatient pain management and wellness practice.

The RN will work closely with the Nurse Practitioner and will provide nursing care in accordance with provider orders, established treatment protocols, and New Jersey nursing standards. Responsibilities will include patient assessment and monitoring, peripheral IV insertion, IV hydration and vitamin therapy, medication administration, electroanalgesia treatments, patient education, and clinical documentation.

The RN must be comfortable working independently within the nursing scope of practice while recognizing when provider consultation or intervention is required.

During periods when the Nurse Practitioner is not physically present, the RN may provide appropriate nursing care pursuant to existing orders and protocols and will communicate with the Nurse Practitioner regarding clinical concerns, changes in patient condition, adverse reactions, or situations requiring a provider-level decision.

Responsibilities

Patient Care & Nursing Assessment

Perform nursing assessments appropriate to the patient’s condition and planned treatment.
Obtain and document vital signs and relevant clinical information.
Review the patient’s current status prior to treatment and identify changes or concerns that may require provider evaluation.
Monitor patients before, during and after treatment.
Recognize and appropriately respond to changes in patient condition.
Provide patient education regarding treatments, medications, hydration and general wellness.
Communicate patient concerns and clinically significant findings to the Nurse Practitioner.

IV Hydration & Vitamin Therapy

Perform peripheral IV insertion and removal.
Administer IV fluids, vitamins, nutrients and medications pursuant to valid provider orders and established protocols.
Monitor patients throughout IV therapy for adverse reactions or changes in condition.
Follow appropriate infection-control, medication-administration and IV safety practices.
Verify medication, dose, route, patient and order prior to administration.
Document all medications, fluids, treatments and patient responses accurately.

Electroanalgesia & Pain Management

Provide electroanalgesia treatments according to the patient’s established treatment plan and protocols.
Properly prepare and position patients for treatment.
Monitor patient tolerance and response to treatment.
Recognize and report unexpected symptoms or treatment concerns.
Document treatment parameters, duration and patient response as required.

Clinical Documentation

Maintain complete, accurate and timely patient records.
Document nursing assessments, treatments, medications, IV therapy, patient education and responses.
Communicate relevant clinical information to the Nurse Practitioner.
Maintain patient confidentiality and comply with HIPAA and Avenix policies.

Clinical Communication & Escalation

Exercise appropriate nursing judgment within the RN’s scope of practice.
Hold or discontinue a treatment when clinically indicated and notify the Nurse Practitioner when provider assessment or a new order is required.
Promptly notify the Nurse Practitioner of significant changes in patient condition, abnormal findings, medication reactions or other clinical concerns.
Activate emergency medical services when clinically indicated and follow emergency procedures.
Do not independently diagnose medical conditions, prescribe medications, or modify provider treatment orders.

Practice Support

Assist with patient scheduling, phone calls and other front-office responsibilities as needed.
Maintain treatment rooms and clinical supplies.
Assist with inventory management and ordering of clinical supplies.
Participate in staff training, quality improvement and development of clinical protocols, as needed.
Maintain a professional, welcoming and patient-centered environment.

Qualifications

Current, unrestricted New Jersey Registered Nurse (RN) license.
Current BLS/CPR certification.
Demonstrated clinical competency in peripheral IV insertion.
Experience with IV medication and/or IV hydration administration preferred.
Strong assessment and clinical judgment skills.
Excellent communication and interpersonal skills.
Ability to work independently while recognizing situations requiring provider consultation.
Strong attention to detail and accurate documentation.
Comfortable working in an outpatient environment.
Pain management, wellness, IV therapy, infusion, neurology or rehabilitation experience is a plus.

Preferred Additional Skills

Experience with peripheral neuropathy, chronic pain or musculoskeletal conditions.
Experience with IV vitamin/nutrient therapy.
Experience with medication administration.
Experience with patient education and outpatient care coordination.
Experience with electrotherapy/electroanalgesia or similar treatment modalities.
Medical front-desk or practice-management experience.

Position Expectations

The RN is expected to practice within the scope of New Jersey RN licensure and Avenix Health and Wellness policies and procedures.

The RN will follow established provider orders and treatment protocols and will use independent nursing judgment when assessing and monitoring patients. Provider-level decisions—including diagnosis, prescribing, creation or modification of medical treatment plans, and changes to medication or treatment orders—remain the responsibility of the Nurse Practitioner or other authorized prescriber.

When the Nurse Practitioner is off-site, the RN may continue providing appropriate nursing care pursuant to existing orders and protocols. The RN is expected to contact the Nurse Practitioner whenever a patient’s condition, treatment request, or clinical circumstance requires provider assessment, clarification, or a new order.
