Salary: $40 per hour

Location:

Southern California: LA, SD, OC, Riverside, Santa Barbara and surrounding area’s.

Job Summary:

Provide concierge in home nursing to clients in the comfort of their homes. Meridian Prime Concierge Services is known for going above and beyond for their patients. We are seeking the top nurses in the field to join our team and work hand in hand with the best professionals in the healthcare industry. This position is for nurses that excel in patient care, customer service, prefer taking care of patients one on one, and form strong connections with their patients. This position is truly for the nurse that loves nursing.

About This Position:

Help manage nursing needs of patients at their home and advocate for their needs.
Provide assistance with all ADLs, medications and nursing needs.
Perform nursing care within the scope of the Vocational Nursing Practice Act and in accordance with the plan of care established by the patient’s physician.
Provide instruction and support to all patients and caregivers regarding the patient’s care and medical status.
Complete and submit required documentation in a timely manner.
Provide care at the patient’s home when needed and as scheduled.
Strong Communication with the agency staff.
Ability to form relationships and maintain rapport with clients, families and referral sources.

Patient/Staff Education:

Demonstrates a commitment to meeting the learning needs of patients and families.
Utilizes appropriate resources to meet those needs and achieve positive patient outcomes.

Leadership/Collaboration/Professional Development:

Demonstrates respect and understanding for peers and other clinical disciplines.
Participates as an effective member of the patient care team to formulate an integrated approach to care.
Demonstrates leadership behaviors and is committed to the development of self and others.

Required Skills:

Previous experience in ICU, ER, PICU, Med Surg, and/or Hospice is preferred.
Current proof of license/registration in the State of California.
Graduate of an accredited school of nursing.
Demonstrated independence in providing clinical care
Demonstrated time management and organizational skills.
Demonstrated active listening skills.
Motivated and works well in an interdisciplinary team approach.
Excellent oral/written communication and interpersonal skills; ability to comprehend written sentences and paragraphs in work related documents.
Must have reliable transportation, valid driver's license, and the minimum state-required liability automobile insurance and is in good working order.

Required license, certification and insurance:

Licensed Vocational Nurse (LVN)
Current CPR certification
NSO Malpractice Insurance ( www.nso.com )
