Full Job Description

Dacotah Foundation is seeking a compassionate, organized and dependable individual for our Full-time Housing Stabilization Case Manager to provide intensive, person-centered services to individuals who have experienced homelessness and are working on maintaining their housing. This position supports housing retention through assessment, individualized service planning, resource coordination, advocacy, crisis response and ongoing engagement with residents, landlords and community partners.

Ideal candidate would be grounded in Housing First, harm reduction, movtivational interviewing, and traum-informed care, with strong documentation skills and a commitment to supporting residents with dignity, respect and consistency.

What You’ll Be Part Of

The Edwinton Place provides ongoing supportive services for individuals meeting tenant criteria navigate their supportive service needs and sustain their housing long-term.

As a case manager for housing stabilization you will be developing person-centered plans and goals with each tenant that identifies opportunities to improve their living environment, health, mental health, social and leisure activities, employment and income sources.

What You’ll Do

Provide ongoing case management services to residents including regular monthly visits
Complete assessments and work with residents to develop individualized per-centered housing plans that foster stability, accountability, and growth
Support residents with lease compliance, housing retention, landlord communication, eviction prevention, and successful tenancy skills
Provide guidance for team members on supporting tenants plans and goals
Connect residents to benefits, healthcare, behavioral health services, substance use suports, employment resources, food assistance, transportation and other community services.
Provide advocacy, supportive cunseling, crisis intervention, safety planning, and referrals as needed
Maintain accurate, timely and confidential documentation, including case notes, service plans, eligibity records, progress updates and required reports.
Participate in team meetings, case consultation, trainings, program evaluation and continuous quality improvement activities.
Collaborate closely with administration, peers, staff, and external partners to ensure seamless, high-quality care
Support new tenants with orientation to the program
Coordinate staffing and ensure program coverage meets required ratios
Facilitate groups, skill-building activities, and individualized support when needed
Work closely with our Program Director to monitor program quality, track outcomes, and contribute to continuous improvement efforts

Qualifications

Bachelor’s degree and/or licensure (Social Work, Nursing, or related behavioral health field)
Minimum of 2 years of relevant experience in mental health and/or substance use services; experience in housing or homeless services may be considered
Experience providing case management, service coordination, housing support, behavioral health support, or related human services preferred
Strong knowledge of homlessness, poverty, trauma, mental health, substance use, disability services, public benefits and community resources preferred
Ability to build trusing professional relationships while maintaining appropriate boundaries and confidentiality
Strong verbal and written communication, problem-solving, advocacy, organization and documentation skills
Ability to work independently in community settings while participating in a collaborative team environment.
Basic computer skills and ability to use case management databases or electronic record systems incluidng HMIS
Possess a valid driver's licens, reliable transportation, proof of insurance and ability to pass required background check preferred
