Full Job Description

Are you a compassionate Social Worker who is passionate about making a meaningful difference in the lives of residents and their families? Join our dedicated team, where you'll play a vital role in providing emotional support, coordinating care, and helping residents navigate every stage of their healthcare journey.

Responsibilities

Conduct resident psychosocial assessments and develop individualized care plans.
Provide counseling, emotional support, and guidance to residents and their families.
Coordinate discharge planning and facilitate smooth transitions to post-acute services.
Collaborate with nursing, therapy, physicians, and the interdisciplinary care team.
Advocate for residents while ensuring their rights, dignity, and well-being are maintained.
Assist with admissions, care conferences, and resident/family communication.
Maintain accurate documentation and ensure compliance with all state, federal, and facility regulations.

Qualifications

Bachelor's or Master's degree in Social Work (BSW or MSW).
Current state Social Work license/certification, as required.
Previous experience in a skilled nursing, rehabilitation, long-term care, or healthcare setting is preferred.
Excellent communication, organizational, and interpersonal skills.
Strong problem-solving abilities with a compassionate, resident-focused approach.
