This rewarding role allows you to deliver exceptional care directly to patients who need it most. You'll combine your wound care expertise with wellness assessments and phlebotomy skills to provide truly comprehensive care to patients in the comfort of their homes. Quick Facts:
- Salary: $110,000-$160,000 annually (based on experience)
- Schedule: Part-time for first 90 days, then full-time (flexible scheduling)
- Setting: Mobile wound care across the region

Responsibilities:
- Conduct comprehensive wound assessments in patients' homes, providing care they wouldn't otherwise access
- Develop and implement individualized treatment plans that drive exceptional healing outcomes
- Perform expert wound debridement procedures with precision and specialized techniques
- Conduct annual wellness check-ups, including preventive screenings that catch potential issues early
- Perform phlebotomy as needed, eliminating the need for vulnerable patients to travel
- Collaborate with an interdisciplinary team to optimize holistic patient care
- Educate patients and their families, empowering them in the healing process
- Document care using our streamlined electronic health record system
- Participate in ongoing professional development to stay at the cutting edge of wound care
Qualifications:
- Current Nurse Practitioner (NP) license in Minnesota (or eligibility for licensure)
- Board certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner
- Experience in wound care and debridement (2+ years preferred, but will consider passionate candidates with transferable skills)
- Wound care certification (WCC, CWCN, CWS) or willingness to obtain within first year (we'll support your certification)
- Comfort with phlebotomy and wellness assessments (additional training provided if needed)
- Excellent clinical judgment and autonomous decision-making ability
- Exceptional interpersonal skills for connecting with diverse patient populations
- Strong problem-solving abilities in varied home environments
- Valid driver's license and reliable transportation
