Full Job Description

Position Overview

Lighthouse Hospice is growing and seeking a compassionate and dedicated Medical Social Worker (PRN) to join our interdisciplinary care team based out of Eagle, Idaho. In this vital role, you will provide emotional, psychosocial, and practical support to patients and their families as they navigate the challenges of serious illness and end-of-life care.

As a Medical Social Worker, you will help ensure that each patient’s journey is met with dignity, respect, and understanding, offering support that honors individual values, cultures, and beliefs. Your clinical expertise and compassionate presence will be key in helping patients and families find comfort, clarity, and peace throughout the hospice experience.

Key Responsibilities

Conduct comprehensive psychosocial assessments for hospice patients and their families.
Provide emotional support, counseling, education, and crisis intervention related to serious illness,caregiving, anticipatory grief, and end-of-life concerns.
Assist patients and families with advance care planning, community resources, financial concerns,and care coordination.
Collaborate with the interdisciplinary team, including nurses, chaplains, hospice aides, physicians,and other healthcare professionals, to develop and implement individualized plans of care.
Participate in Interdisciplinary Group (IDG) meetings as requested.
Respond to urgent psychosocial needs and provide timely interventions when appropriate.
Complete accurate and timely documentation in the electronic medical record in accordance withagency standards and Medicare Conditions of Participation.
Maintain professional communication with patients, families, facilities, referral sources, and members of the hospice team.

Qualifications

Bachelor’s or Master’s degree in Social Work from a CSWE-accredited program.
Current Idaho Licensed Social Worker (LSW) or Licensed Master Social Worker (LMSW) license in good standing.
Minimum of one year of social work experience; hospice, palliative care, home health, or healthcare experience preferred.
Valid driver’s license, reliable transportation, and current automobile insurance.
Strong assessment, communication, organization, and documentation skills.
Ability to work independently while maintaining effective communication with the interdisciplinary team.

Additional Preferred Qualifications

Previous hospice or palliative care experience.
Current Oregon social work license (or eligibility to obtain one) is preferred but not required.
Experience using an electronic medical record.

Schedule & Availability

This is a PRN (as-needed) position. Hours are not guaranteed and will vary based on patient census, staff vacations, and agency needs.

The ideal candidate must have availability during Monday through Friday business hours, as this is when the majority of patient visits occur. While occasional weekend coverage may be requested, it is infrequent at this time.

Flexibility to accept visits with advance notice, and occasionally on shorter notice, is highly valued.
