Job Title: Nurse Practitioner Anchor

Location: 1575 Blue Hill Avenue, Mattapan, MA 02126

Reports To: FNP Anchor Program Director

Type: Full Time

Summary:

Mattapan Community Health Center is building a world class primary health care system that is committed to caring for special populations, and that is focused on improving health outcomes for our patients as well as building healthy communities. In this role NP Anchors will provide comprehensive primary care services in an ambulatory setting serving a diverse and underserved community.

This is a two-year structured program designed to support the transition to independent practice. The program will be intentionally phased to allow for progressive skill development and increasing provider-level responsibility. The initial phase of the NP Anchor program will focus on strengthening their fundamental outpatient and nursing-based clinical skills such as patient triage, care coordination, and ambulatory workflow. As the program progresses, the NP Anchors will transition to providing direct provider-level care under close preceptor supervision. This includes independently completing patient assessments and evaluations, formulating diagnoses and plans.

Upon completion of the program, NP Anchors are expected to demonstrate the ability to provide competent, high-quality patient care to complex patient populations. The NP Anchors will have learned how to effectively manage time and workload. They will be able to proficiently manage full patient panels. The NP Anchors will have enhanced leadership ability and mastered the ability to delegate to the clinical support staff.

Responsibilities:

.25 FTE Registered Nurse

Clinical Duties

Provides direct patient care, utilizing the nursing process.
High complex telephone and in-person triage
Assesses patients including vital signs, height, weight, BMI, allergies, medications and reason for visit
Performs procedures such as the administration of medications either orally or by injection and immunizations as ordered, PPDs, suture removal, wound care, dressing changes, ear irrigation, EKG’s, visual acuity screening, audiometry, O2 sats, blood glucose per Hemocue protocol, peak flow, nebulizer treatments, etc.
Collects specimens from patients for laboratory tests and notifies requesting providers of abnormal results.
Perform lab testing such as urine pregnancy test, rapid streps and routine urinalysis. and rapid strep test using correct protocols and procedures.
Triages patients by telephone or directly as walk-ins to identify urgency and level of care needed. Makes appropriate referrals required for the management of patient care.
Assists providers with procedures such as suctioning, I&D, IUD insertion, endometrial biopsy, straight catheterization, Colposcopy, suturing
Provides or arranges for patient education, such as instructing patients in the proper use of medical equipment, medication administration, diabetic teaching and other nursing care regimens.
Ensures that follow-up is made when patients miss scheduled appointments in order to determine reason and reschedule.
Assures appropriate scheduling and assists with patient flow.
Provides translation as needed.
Assist providers with calling patients regarding abnormal labs, sending medication refills and paperwork at the request of the provider

Administrative Duties

Records in patient’s medical record all care provided and ensure that appropriate forms are properly completed for billing purposes.
Monitors needs for equipment repair/replacement and notify nursing supervisor.
Participates in individual or group teaching exercises.
Precepts nursing and medical assistant students as needed.
Provides follow-up on patients seen in the ER, hospital and, patients with chronic diseases and abnormal labs.
Works on special programs/projects as assigned.
Assigned to work with a NP or MD and assist with paperwork, medication refills or other tasks asked to perform

.75 FTE Nurse Practitioner Anchor

Clinical Duties

Weekly check ins with preceptor to discuss complex cases
Overall direction and integration ambulatory and chronic care as well as sub-specialty and rehabilitation services through management of first-contact, continuous, comprehensive, and efficient health-care services for a defined group of patients
Medical evaluation, diagnosis and treatment of assigned MCHC patients.
Management of urgent clinical events
Referral of patients to appropriate clinical facilities, consultants and / or social service providers as indicated by good medical practice.
Maintenance of current, accurate and appropriately detailed records of medical visits in official MCHC’s medical charts using the SOAP format.
Participation in maintenance of various clinical database(s).
Provision clinical cross-coverage for patients assigned to other MCHC providers.
Maintenance of competence in all diagnostic and/or surgical procedures that he / she is qualified and plans to perform at MCHC in the course of practice.
Provision patient education in individual and group settings
Focus on health promotion and disease prevention, including appropriate screening and preventive services
Assistance of clinical staff with the daily operations (e.g. patient flow, walk-in triage, cross-coverage issues)..

Administrative Duties

Attend all departmental meetings, all-staff meetings and providers meetings except when on leave or excused by the Medical Director.
Participate with other staff in quality improvement, utilization review and other agency audits.
Participate in committees as designated.
Assist in the development of practice guidelines, clinical protocols, or other educational programs.
Participate in off-site MCHC and affiliated agency / institution activities as necessary.
Perform other related duties as assigned by the Medical Director.
Participate in weekly supervision with the Medical Director as defined in his / her contract.
Is responsible for clinical supervision of all nursing staff working directly with his / her during the daily practice of patient care.

Performs other related duties as required or assigned by supervisor.

Requirements:

Qualifications:

Education – Active Registered Nurse License in Massachusetts. Current certification in Cardio-Pulmonary Resuscitation (CPR). Graduate from an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program with a specialty as a Family Nurse Practitioner. Licensed (APRN) and credentialed (FNP-C or FNP-BC) – or license/credential-eligible as FNP by program start date (September).
Clinical Knowledge – The ideal candidate must have one year of recent clinical experience in a hospital or community health care setting, must be flexible, organized, and able to stand on their feet for extended periods of time and be culturally/ethnically sensitive. Experience in a community health setting with persons of diverse cultures is a plus.
Communication and Interpersonal Skills - Bilingual in Haitian-Creole or Spanish preferred. Comfort utilizing medical interpreter services required. ideal candidate will have excellent communication skills, both written and verbal, to effectively interact with staff, physicians, patients, and external stakeholders.
Experience – At least six months of experience as a Registered Nurse. Three or more years of experience working with a diverse population in a health care setting is preferred.
Analytical Skills – Advanced computer operations skills, such as Excel and Microsoft Office, are essential. Experience with EPIC OCHIN is a plus.
Adaptability and Resilience – Able to commit to program length of two (2) year minimum. The ability to thrive in a fast-paced and evolving healthcare environment, adapt to changing priorities, and manage multiple projects simultaneously.
Ethical and Professional Conduct - Demonstrated commitment to ethical practices, patient-centered care, and maintaining confidentiality.
