Full Job Description

At South Shore Health, we come together to improve the health of our family, friends and neighbors by bringing together people, caregivers and excellence in medicine. We are seeking a Behavioral Health NURSE Navigator to collaborate with providers, nursing colleagues, and partnering healthcare service providers to facilitate bed placement for patients with psychiatric and medical comorbidities. The ideal candidate will be an RN with acute care ED or Med-Surg experience who can coordinate care for patients with comorbidities who are not prepared for medical discharge. If you are a skilled individual with a solid understanding of psychiatric and medical diagnoses who can work with multiple stakeholders to ensure appropriate placement and care, we invite you to join us. The annual pay range for this role is $93,000 - $139,800.

Responsibilities:

· Deliver behavioral health coaching (e.g., brief intervention), remove barriers to care (stigma, insurance status), navigate patients to long-term behavioral healthcare, educate and empower clients on behavioral health intervention and screening, and advocate for and coordinate client care with PCPs and behavioral health providers

· Assess presenting problems efficiently, develop initial treatment plan with the client or family seeking treatment, and assist clients in understanding their diagnosis, treatment options, and the resources available, including educating eligible clients about appropriate community services, clinical research studies, and technologies

· Complete intake of high-risk clients, working with the client, family and other members of the healthcare team as needed to assess client’s behavioral health needs, physical health, support system, financial resources, and available community and government resources

· Determine objectives, goals, and measures that meet the client’s needs and that have been identified through assessment and find a mental health home for the client and provide warm hand-offs to long-term mental health providers

· Collect and chart data, track outcomes, support strategic planning processes and assure collection and management of Behavioral Health data from PCP clinics

· Provide administrative oversight and support for collaborative partnerships by troubleshooting implementation processes and resolving program issues with all partners and stakeholders

· Participate in continued learning and possess a willingness and ability to learn/utilize new technology and procedures to continue developing in your role and throughout the organization

Qualifications:

· BSN required; MSN preferred

· RN-Registered REQUIRED ; RN-BC Psych certification preferred

3-5 years of nursing experience in behavioral health

· Strong working knowledge of local and state regulations and local community resources

· Experience using the Massachusetts PointClickCare and EDI bed search system strongly preferred.
