Full Job Description

Position Title: Social Work and Behavioral Health Manager
Department: PACE
Supervisor: Director of Clinical Services
Hours per week: Full Time; 1.0 FTE
FLSA Status: Exempt

Pay Range: The pay range for the position is $90,000 - $120,000, commensurate with experience and licensing; including other factors, such as language, certifications, etc.

Primary Function:
The PACE Social Work and Behavioral Health Manager oversees the delivery of social work and behavioral health services within the Program of All-Inclusive Care for the Elderly (PACE). This position ensures high-quality, person-centered care by supervising social work staff, coordinating behavioral health resources, supporting interdisciplinary team (IDT) collaboration, and promoting participants’ psychosocial well-being, independence, and ability to remain safely in the community.

Duties & Responsibilities:

Provide supervision, guidance, and performance management to social work staff, including hiring, training, and evaluation.
Ensure delivery of comprehensive assessments and individualized care plans for participants.
Carry a caseload of participants to support program operations.
Participate in IDT updates, care plan meetings, and ongoing care coordination.
Collaborate with the interdisciplinary team (IDT) to develop, implement, and monitor care plans
Support staff in addressing complex situations, including mental health, housing instability, caregiver stress, and end-of-life planning.
Ensure compliance with PACE regulations, documentation standards, and organizational policies.
Monitor quality outcomes and implement improvements related to social work services.
Facilitate team meetings and case conferences as needed.
Provide crisis intervention and support for participants and families.
Promote participant rights, dignity, and culturally competent care.
Identify and help present cases to the inter-PACE organization ethics committee.
Coordinate with community resources to support participants’ social, emotional, and environmental needs.
Participate in program development, audits, and accreditation activities.
Collaborate with behavioral health providers (internal and external) to support participant mental health needs.
Ensure timely and accurate EMR documentation and reporting.

Minimum Basic Knowledge:

Knowledge of the PACE model of care and interdisciplinary team approach.
Understanding of aging-related needs, including chronic illness, cognitive impairment, and functional limitations.
Basic knowledge of mental health conditions, substance use, and trauma-informed care.
Familiarity with community resources, public benefits, and entitlement programs (e.g., Medicaid/MassHealth, Medicare).
Understanding of care planning, case management principles, and person-centered practices.
Knowledge of applicable federal and state regulations related to PACE and long-term care.
Awareness of cultural competency, health equity, and social determinants of health.
Basic computer skills and familiarity with electronic medical records (EMR) systems.

Experience & Qualifications:

A Master’s degree in Social Work from an accredited program with current LICSW licensure in the Commonwealth of Massachusetts preferred (or LICSW within 6 months of employment start).
At least 1 year of experience working with older adults or in a healthcare setting.
Prior supervisory or leadership experience.
Strong knowledge of aging, chronic illness, mental health, and community-based services.
Experience working within interdisciplinary teams.
Excellent communication, organizational, and problem-solving skills.
Ability to manage multiple priorities in a fast-paced environment
Familiarity with PACE programs, Medicare/Medicaid, and long-term care systems preferred.

Essential Functions:

Provide clinical leadership to the Social Work Department and serve as the primary subject matter expert for behavioral health within the PACE program.
Ensure social work and behavioral health interventions are aligned with PACE principles, regulatory requirements, and evidence-based practices.
Maintain communication with participants, caregivers, and families to ensure understanding of care plans, services, and available support.
Assist in identifying high-risk care concerns and develop targeted interventions in collaboration with the IDT.
Serve as a resource for crisis de-escalation and complex case management.
Ensure timely follow-up on reportable events, grievances, or concerns related to social work or behavioral health.
Safeguard confidentiality and protect participant health information in compliance with HIPAA and PACE requirements.
Support staff development through ongoing education, mentoring, and competency monitoring.
Represent the Social Work/Behavioral Health department in internal committees, workgroups, and initiatives.

Physical Requirements:

Able to sit, stand, and walk for extended periods; move throughout office, community, and residential settings; and occasionally lift or carry up to 30 pounds. Position requires frequent use of computers and office equipment, as well as the ability to travel locally for home visits, meetings, and community outreach.
Must be able to communicate effectively in person, by phone, and in writing. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

Supervisory Responsibility:
Clinical and administrative supervision of social work staff.

Define Access Level to PHI: Level 4: Authorized to access full health information. UCHC position and job responsibilities involve the provision of patient care and working as a clinical team member. Staff in this category level, although allowed full access, should only access the necessary information for each respective treatment encounter/circumstance.
