Job description:

JOB TITLE: Case Manager

REPORTS TO: Clinical Supervisor

DESCRIPTION: The case manager in a substance use treatment program is a vital member of a multidisciplinary treatment team. Case managers work closely with patients to ensure that outside matters are handled efficiently and in accordance with the patient’s wants and needs.

DUTIES AND RESPONSIBILITY:

Serve as the primary point of contact for patient advocacy, including navigating insurance, legal and medical appointment concerns.
Schedule initial meeting with patient no more than 24 hours or next business day following patient’s arrival at Hillside for ongoing assessment, treatment planning and monitor compliance with treatment plan, adjusting treatment plan when necessary.
Act as advocate and liaison for participants in securing access to community services and supports.
Create, explain, and submit proper documentation for FMLA, PFML and any other work related paperwork.
Knowledge of state assistance programs, including SNAP, cash benefits, MassHealth, and other available resources for low-income individuals and families.
Complete and maintain chart documentation following program standards and timelines.
Ensure HIPAA compliance while collaborating with outside agencies.
Lead psycho-education groups on addiction, recovery and coping skills.
Case management for HIV+ participants, coordinate and communicate with PCPs and other healthcare providers.

QUALIFICATIONS

High School or GED and or 2 years’ experience with the target population.
Bachelor’s Degree in human services or related field, and/or 1+ years of providing services to the target population.
Knowledge of PFML, FMLA, Short Term Disability
Knowledge of assistance programs; i.e SNAP, cash benefits, SSDI, MassHealth
Extensive experience with EMR (KIPU)
Strong verbal and written communication skills
Critical thinking skills while under pressure
Able to work with patients with a wide variety of needs
Ability to manage multiple priorities and use deescalation skills
Ability to work independently and under time constraints

Hours of Availability

Saturday-Wednesday or Wednesday-Sunday or Thursday- Monday

Available shifts: 9:00 AM – 5:00 PM, 10:00 AM – 6:00 PM, or 11:00 AM – 7:00 PM
