Our services include Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI), and other value-based care initiatives designed to help patients better manage chronic conditions and improve health outcomes.

We are seeking a compassionate, organized, and clinically skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our growing Hybrid CCM & BHI team. This role is ideal for nurses who enjoy building meaningful patient relationships, coordinating care, and helping patients navigate both physical and behavioral health challenges.

Position Summary

The Hybrid CCM & BHI Nurse will provide on-site care management services to Medicare patients enrolled in Chronic Care Management (CCM) and Behavioral Health Integration (BHI) programs. This role combines chronic disease management with behavioral health support, requiring strong communication skills, care coordination expertise, and a patient-centered approach.

The nurse will collaborate with primary care providers, behavioral health providers, psychiatric consultants, caregivers, and community resources to improve patient outcomes and overall well-being.

Key Responsibilities

Chronic Care Management (CCM)

Conduct monthly on-site care management services to patients with chronic conditions
Complete comprehensive health assessments
Develop, update, and monitor individualized care plans
Perform medication reconciliation and adherence reviews
Coordinate care among providers, specialists, and caregivers
Educate patients on chronic disease self-management
Identify barriers to care and connect patients with appropriate resources

Behavioral Health Integration (BHI)

Conduct monthly on-site behavioral health screenings and assessments, including PHQ-9 and GAD-7
Monitor behavioral health symptoms and treatment progress
Develop and maintain behavioral health care plans
Support patients experiencing depression, anxiety, grief, stress, and other behavioral health concerns
Coordinate care with behavioral health providers and psychiatric consultants
Escalate urgent behavioral health concerns according to established protocols

Documentation & Compliance

Document all patient interactions accurately and timely
Maintain compliance with CMS CCM and BHI requirements
Track and document time spent on billable activities
Ensure documentation supports billing and audit readiness
Participate in quality assurance and compliance initiatives

Required Qualifications

Active and LPN or RN Massachusetts license
Minimum of 1 year of nursing experience
Strong assessment, communication, and care coordination skills
Experience working with geriatric, chronic care, or behavioral health populations
Proficiency with EMR/EHR systems and remote technology
Ability to work independently while collaborating within a remote team environment
Excellent verbal and written communication skills

Preferred

Chronic Care Management (CCM) experience
Behavioral Health Integration (BHI) or Collaborative Care Model (CoCM) experience
Case Management or Care Coordination experience
Long-Term Care, Primary Care, Geriatrics, or Population Health experience
Previous remote nursing experience
Experience administering PHQ-9, GAD-7, and other behavioral health assessments

Hybrid Work Requirements

Ability to communicate to partnered nursing home facilities in the Hyde Park area.
Computer capable of supporting company-approved software and security requirements
Ability to maintain patient confidentiality in a hybrid and remote environment

Compensation

$30.00 – $45.00 per hour

Compensation is based on licensure, experience, care management background, and overall qualifications.
