Registered Nurse (RN) –

Hospice Care – Routine, Admissions, Supervisory & Recertification Visits

We are a compassionate and dependable hospice agency seeking a Registered Nurse to support our growing team in South County San Diego. This role is ideal for an independent, experienced RN who is comfortable working autonomously and driving to patient homes and facilities throughout the region.

We are looking for someone to assist with routine nursing visits, supervisory visits, recertification assessments, and new admissions as needed. The position is per diem (as needed), offering flexibility to fit your schedule while still making a powerful impact in the lives of patients and families during their most important moments.

We pride ourselves on maintaining a family-owned feel while providing exceptional, mission-driven care. Our team supports each other with reasonable caseload expectations, admission nurse assistance, and float support.

Our Ideal RN Candidate Is:

Flexible and willing to travel throughout San Bernardino County
Independent, organized, and able to manage their time effectively
A strong communicator with excellent assessment and critical thinking skills
Compassionate and deeply committed to the hospice mission
Open to feedback and thrives in a team-oriented culture

Essential Responsibilities:

Perform routine, supervisory, recertification, and admission nursing visits as assigned
Conduct in-home and facility assessments to evaluate patient status and needs
Develop individualized care plans and provide education to patients and caregivers
Administer medications and manage symptoms in accordance with hospice protocols
Communicate and coordinate care with the interdisciplinary team and referral partners
Complete timely, accurate documentation in the EMR

Qualifications:

Current California RN license
Current CPR certification (AHA BLS-compliant)
Valid driver’s license and auto insurance
Reliable transportation and willingness to drive throughout the county
Ability to lift and assist patients as needed
