Full Job Description

NOTE: This is a hybrid position, however, residence in Illinois St. Louis area. Responsibilities will be split between working from home (75%) and completing in-person Telehealth visits in the field (25%).

We are looking for a highly motivated and energetic Care Manager to join our team. The ideal candidate for this role is a CMA, LPN, or RN. who is experienced and comfortable with Principal Care Management and Chronic Care Management workflows. We focus heavily on coordinating care for our patients as well as improving patient outcomes in the post-acute space.Responsibilities:

Clinical Chart Review & Monitoring: Review EMR data and vitals dashboards to identify trends, gaps in care, or deviations from baseline.
Care Plan Development & Updates: Create, update, and maintain individualized care plans based on clinical data, diagnoses, and provider/SNF protocols.
Clinical Escalation Management: Initiate timely escalations to SNF clinical staff when vitals or clinical indicators fall outside defined parameters.
EMR Documentation & Flagging: Accurately document findings, alerts, interventions, and escalations within the EMR, ensuring audit-ready and compliant records.
Technology-Driven Risk Identification: Use care management platforms, rules engines, and alerting tools to proactively flag high-risk patients and priority issues.
Interdisciplinary Coordination : Communicate clinical insights and recommendations to SNF teams and internal care teams.

Requirements:

CMA, LPN OR qualifying medical certification
High School diploma or equivalent
Preferred: 2+ years of post-acute healthcare experience (Skilled Nursing Facility and/or Home Health Agency)
Advanced computer skills
Electronic messaging communications skills
Ability to work with multiple computer screens and programs simultaneously
Positive attitude and ability to work well with others in a team environment
Preferred: Previous Remote Patient Monitoring or PCM/CCM experience

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
