Full Job Description

Job Overview
We are seeking a motivated and compassionate Nurse Practitioner to join our healthcare team, dedicated to delivering exceptional patient care across diverse clinical settings. In this role, you will utilize your advanced nursing skills to assess, diagnose, and manage patient health needs while collaborating with physicians and other healthcare professionals. Your expertise will help improve health outcomes, promote preventive care, and support patients through their health journeys. This position offers an exciting opportunity to work in a fast-paced growing and innovative new primary care health care clinic environment where your clinical knowledge and proactive approach will make a meaningful difference.

Responsibilities

Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews.
Develop and implement individualized treatment plans in accordance with best practices and current clinical guidelines.
Administer medications, including injections and IV infusions, following proper protocols and wound care.
Manage acute and chronic care conditions in the primary care clinic setting as well as Skilled nursing at local nursing home.
Utilize electronic health record (EHR) systems like Medconnect, preferrred but not required for accurate documentation of patient encounters and medical records.
Collaborate with multidisciplinary teams on transition of care and utilization review to ensure comprehensive patient care.

Qualifications

Active Alabama Nurse Practitioner license.
Proven experience in diverse clinical environments such as primary care, hospital medicine and/or SNF with wound care experience, preferred not required.
Strong knowledge of medical terminology, physiology, ICD coding (International Classification of Diseases), CPT coding (Current Procedural Terminology), and HIPAA regulations.
Hands-on experience with EMR/EHR systems including Medconnect, preferred but not required.
Skilled in performing procedures for wound care preferred but not required.
Strong clinical judgement and communication skills.
Knowledge of pharmacology related to primary care.
DEA license preferred or the ability to obtain.
