Full Job Description

Overview

New England Hospice is seeking a compassionate, organized, and detail-oriented Intake Coordinator to oversee all aspects of the referral and intake process. The Intake Coordinator serves as the first point of contact for referral sources, patients, and families, ensuring a smooth transition into hospice services while maintaining compliance with all regulatory and accreditation requirements.

This role is responsible for managing referrals, coordinating required documentation, verifying insurance eligibility, facilitating timely admissions, and building strong relationships with referral partners and community providers.

Duties

Manage all daily hospice referral and intake operations.
Receive, process, and track referrals to ensure timely admission of eligible patients.
Coordinate and obtain all required documentation, including physician orders, demographics, insurance information, and supporting clinical records.
Verify insurance coverage and obtain authorizations as required.
Maintain compliance with federal, state, HIPAA, and ACHC regulations and standards.
Communicate effectively with hospitals, physicians, facilities, case managers, and other referral sources.
Provide education regarding hospice services to patients, families, and referral partners.
Ensure a seamless transition of care from referral through admission.
Maintain accurate referral statistics and generate intake reports as requested.
Develop and maintain positive relationships with referral sources and community partners.
Assist in identifying opportunities to improve services and meet community needs.
Participate in quality improvement initiatives and performance improvement activities.
Maintain knowledge of community resources and assist referral sources when services fall outside the hospice scope of care.
Perform other duties as assigned.
