Full Job Description

Advanced Center for Nursing and Rehabilitation is currently seeking an MDS Coordinator to join its team.

Qualifications:

Up to date CT LPN or RN license
organization skills
ability to prioritize work
strong communication skills; ability to effectively communicate with staff, families and residents in a professional and tactful manner.
Ability to perform duties in a professional manner
Ability to work as a team
Experience with OBRA assessments, Medicare PDPM and Case Mix Index
Knowledge of and compliance with HIPAA guidelines
Current with changing regulations as they pertain to clinical reimbursement and coding

Job Responsibilities:

Gather data and complete Resident Assessment Instrument (RAI) in an accurate and timely manner.
Coordinate efforts from all resources probable, such as residents, staff, health care professionals and the care plans to achieve optimal plan for each individual plan of care.
Prepare MDS for electronic transfer to the state.
Ensure that the MDS and completed care plans are well within the regulatory guidelines.
Ensure the complete plan of care produced is clear, concise and pertinent to each individual resident.
Completion and implementation of the plan of care.
Accurately document resident direction and achievement according to the plan of care.
Effectively communicate with the DNS of any issues or concerns regarding completion of the MDS/Care plan and concerns regarding direct resident care.

Job Type: Full-time

Pay: $104,000.00 - $110,000.00 per year
