Full Job Description

The Housing Case Manager's goal is to provide housing case management services to sixteen tenants living at Avalon Place apartments in order to help the tenants achieve housing stability. The Housing Case Manager will be expected to work with a diverse population and must be sensitive to the culture, religion, language, age, gender, and sexual orientation of the population served.

ESSENTIAL DUTIES

· Engage tenants in services.

· Assess tenant strengths and obstacles to housing success.

· Ensure tenants’ safety is assessed and a safety plan is developed.

· Assist tenants with developing adult daily living skills (ADLS).

· Assist tenants with securing and maintaining funding sources.

· Assist tenants with locating primary care providers and assist in addressing medical health issues.

· Help tenants connect with basic resources in the community, ie: food bank, shopping, and clothing needs.

· Coordinate mental health services with mental health case managers at TR or other community mental health case managers.

· Develop a housing success plan with each tenant, record service delivery, maintain housing charts and service documentation.

· Participate in staff meetings.

· Develop group activities that will enhance the tenant’s abilities to live independently.

· Work collaboratively with The Director of Supportive Housing and Front Desk staff to help maintain safety and promote tenant success while living at Avalon Place apartments.

· May be required to perform light janitorial or maintenance duties as appropriate.

· Tolerate unruly client behavior.

· Assist with other duties as assigned.

· Flexibility to assist with other housing programs as needed.

· Must be able to work on-site, remote work is not available for this position.

RELATIONSHIP WITH COMMUNITY AND CO-WORKERS

· Contributes to cooperative working relationships with other agency staff and with supervisors.

· Solicits and provides case consultation and coordination as needed.

· Presents a positive professional image of the agency to community resources utilized by the tenants of Avalon Place apartments.

MINIMUM QUALIFICATIONS

· B.A./B.S. degree in human service field

· Two years experience working with homeless and low-income individuals, including persons living with mental and physical illnesses or chemical dependency/addictions

· Experience working with people living with serious mental illness or co-occuring disorders is preferred

· Valid Washington state drivers license

· Ability to pass a state criminal background and fingerprint check

· Ability to walk up and down multiple flights of stairs and lift up to 30 pound if needed.

COVID-19 REQUIREMENTS

The agency follows CDC and Washington State Department of Health’s guidelines in its measures to prevent the spread of COVID-19. Must be able/willing to wear a mask if needed during an outbreak of Covid19 at the program. Must be willing to work with clients who may be positive for Covid19 and may be unwilling to wear a mask.

BENEFITS

Benefits include 22 paid leave days per year of service and 10 paid holidays (each pro-rated per fte); medical and dental insurance; long term disability insurance; voluntary participation in one of the Agency sponsored 403(b) retirement plans and AFLAC cafeteria plan.

HOURS

40 hours per week, Monday through Friday.
