Case Manager — Mass Bay Behavioral Health (Dorchester, MA)

Job Type: Full-time, In-person
Pay: $50,000 – $55,000 per year

About Us
Mass Bay Behavioral Health is an outpatient behavioral health provider offering Partial Hospitalization (PHP) and Intensive Outpatient (IOP) programming in Dorchester, MA. Our team delivers structured, compassionate, evidence-based care to adults navigating mental health and substance use challenges. We're looking for a Case Manager who is organized, detail-oriented, and genuinely invested in helping clients stay connected to treatment without losing their footing at work or home.

About the Role
The Case Manager coordinates care for clients across all levels of programming — and plays a central role in protecting clients' employment and income while they're in treatment. A significant part of this position involves completing and managing leave-of-absence documentation, including Massachusetts Paid Family and Medical Leave (PFML), FMLA, and short-term disability paperwork. Accuracy, timeliness, and follow-through on these forms directly affect whether our clients get paid and keep their jobs while they focus on recovery. Responsibilities

Leave & Disability Coordination (core function)

Complete, submit, and track client leave documentation, including MA PFML applications, FMLA certifications, and short-term/long-term disability forms
Coordinate with the clinical team to obtain required provider certifications and clinical information supporting medical necessity for leave
Communicate with the Department of Family and Medical Leave (DFML), employers, HR departments, and third-party leave administrators on clients' behalf (with appropriate releases)
Track deadlines, recertification dates, and extension requests to prevent lapses in client benefits or income
Educate clients on their leave rights, application process, and required documentation at intake and throughout treatment

Care Coordination

Coordinate services for a caseload of PHP/IOP clients from intake through discharge
Develop and execute aftercare plans, including step-down referrals, outpatient linkages, and community resources
Connect clients with housing, transportation, benefits (MassHealth, SNAP, etc.), and primary care
Document all case management activity accurately and timely in Kipu EMR
Participate in treatment team meetings and communicate client needs, barriers, and progress
Monitor attendance and engagement; conduct outreach to clients at risk of disengaging
Maintain compliance with 42 CFR Part 2, HIPAA, and all applicable confidentiality regulations

Qualifications

High school diploma or equivalent required
Bachelor's degree in social work, psychology, human services, or a related social science field preferred
Relevant experience in behavioral health, case management, or human services required
Experience completing PFML, FMLA, or disability paperwork strongly preferred — direct experience with the MA DFML portal is a major plus
Exceptional attention to detail and deadline management; comfortable owning time-sensitive administrative processes
Experience with Kipu EMR or comparable electronic medical record systems a plus
Strong written documentation and client communication skills
