Full Job Description

The Credentialing Coordinator is assigned the responsibility of assisting the Medical Staff Services/Academic Affairs in the coordination of all medical staff activities. To ensure compliance with all regulatory and accreditation agency credentialing standards as well as the Medical Staff Constitution and Bylaws, the primary responsibility of the Credentialing Coordinator will include the collection and verification of relevant information including current competence, conduct and malpractice history, which will serve as the basis for decisions regarding reappointment to the medical staff. PRINCIPAL DUTIES AND RESPONSIBILITIES: RATING 1 – 3 RESPONSIBILITIES (See Page 3) Promote a safe, cooperative and professional health care environment to ensure optimum patient care (as per HR Policy 113c). Adheres to all hospital policies, procedures, rules and regulations, including but not limited to, absenteeism, ETIME, cellular telephone usage, dress code and rules of conduct. Makes Customer Service a priority, treating customers (patients, visitors, and co-workers) in a professional manner exercising courtesy and tact. Effectively communicates with patients and others about SBH services in their area(s) of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in SBH in a timely fashion. Reports to the Manager/Director of Medical Staff Services/Academic Affairs Maintains individual credential file for each practitioner. Maintains provider information in appropriate entity in multi-entity computer database. Implements the credentialing process for reappointment according to established procedures. Following established policy, obtains temporary privileges when needed. Collects malpractice history information via inquiries to other facilities and/or inquiry to malpractice carrier and correlates information. Verification of practitioner status with agencies such as NYS Office of Professional Medical Conduct (OPMC), Office of the Inspector General NYS Department of Social Services/Medicaid as outlined in established policy Primary Source verification of documents (i.e., license, DEA, Board Certification) as outlined in established policy. Queries National Practitioner Data Bank and supervises input of information into computer database. Provides data to Quality Assurance Committee regarding all initial appointments, permanent appointment, reappointments, termination and resignations. RATING 1 – 3 RESPONSIBILITIES (See Page 3) Collection of information required for reappraisal of competency from appropriate committee/department (i.e., Infection Control, Risk Management, M & M Committee, Quality Assurance Committee, Member Services/ Patient Complaints). Upon receipt of all required information and verifications, will obtain appropriate signatures required prior to presentation to Credentials Committee for review and approval Ensures that credential and quality assurance files are presented to the Credentials Committee and appropriate signatures are obtained prior to presentation to the Medical Board for review. Preparation of Report to the Board of Trustees Following approval by Board of Trustees, appropriate signatures will be obtained, and practitioner will be notified of their status. Provides report on actions taken by the Board of Trustees to appropriate staff and administrators. Provides support to Credentials Committee and Medical Board. Support will consist of scheduling, agenda preparation, announcements, attendance, and preparation of minutes and distribution of materials/information as required. Acts as liaison with managed care organizations who delegate credentialing to the Hospital and participates in credentialing review. Provides information to organization within the St. Barnabas umbrella regarding physician information as per delegated credentialing agreements. Acts as a resource person for facility personnel and provides information as requested. Supervises preparation of all mailings. Prepares for the annual meeting of the Medical Staff including caterer, announcements, awards, and agenda. Assists in the maintenance of Corporate provider database. Assists in preparation of all regulatory and accreditation surveys. Works to ensure that medical staff and/or provider lists are kept current. Directs and assists with all departmental activities as needed, including but not limited to maintaining supplies in the Doctors' Lounge and coordinating award ceremonies. Prepares for and participates (when needed) in all surveys conducted by any state, federal regulatory or accreditation organization. Initiates requests for malpractice coverage through the SBH Qual PPLMP program for new and current providers, as requested and agreed upon by the department. The above statements reflect the general duties considered necessary to describe the principal functions of the job as identified, and shall not be considered as a detailed description of all work requirements that may be inherent in the job. QUALIFICATIONS: A High School Diploma is required. The candidate must possess excellent computer skills and strong communication abilities, both verbal and written. The role demands the ability to handle sensitive information with discretion, tact, and diplomacy, as well as the capability to work independently with minimal supervision. Working Conditions: Involves managing responsibilities across multiple facilities, with frequent interruptions and a demanding, time-sensitive workload.
