Full Job Description

Position Summary:

The Case Manager is a frontline outreach and engagement professional supporting NCS’s PATH-funded Chance to Connect programs. These initiatives serve literally and chronically homeless adults in Manhattan, particularly those with serious mental illness (SMI), substance use disorders, or co-occurring disorders (COD), by offering low-barrier, trauma-informed, and culturally competent support services that facilitate access to housing, treatment, and long-term recovery.

The Case Manager provides outreach, engagement, needs assessments, service referrals, follow-up, and documentation for PATH-eligible individuals encountered primarily at Upper East Side meal programs and at the Chance to Connect resource center. The role emphasizes trust-building, person-centered planning, and linking clients with appropriate supportive services.

Essential Duties and Responsibilities:

Conduct regular outreach at community meal programs, including St. James’ Church, Madison AvenuePresbyterian Church, and Church of the Epiphany, to identify and engage PATH-eligible clients.

Establish rapport and build trust with individuals who are service-resistant or disconnected from formalsupport systems.

Use a harm reduction and trauma-informed approach to engage clients with complex behavioral healthand housing needs.

Conduct initial eligibility screenings and informal assessments of mental health, substance use, housing, and basic needs.

Provide ongoing case management including assistance with:

Accessing shelter or drop-in services
Applying for housing via the 2010e and coordinated entry
Connecting to mental health and substance use treatment (e.g., Chance for Change, CUCS, SUSACT Teams, etc.)
Navigating benefits (e.g., SSI/SSDI, Medicaid)
Referrals to medical, psychiatric, vocational, and legal services

Provide on-site case management at Chance to Connect, NCS’s low-threshold service hub housed at Madison Avenue Presbyterian Church.

Job Description – CASE MANAGER, CTC – continued

Support walk-in clients with crisis intervention, basic needs, harm reduction supplies, hygiene kits, and connection to services.
Collaborate with co-located providers, including the Chance for Change OASAS-licensed outpatient program, to offer warm handoffs and integrated services.
Maintain accurate, timely documentation of all client contacts, services provided, referrals made, and follow-ups in the AWARDS EHR system.
Complete and maintain PATH eligibility documentation and client-level data as required by SAMHSAPATH guidelines.
Assist in collecting program metrics for quarterly and annual reports, including outreach logs and service outcome tracking.
Coordinate care with external providers including medical clinics, ACT teams, housing placement providers, and shelters.
Participate in weekly supervision, team meetings, and periodic case conferencing with interdisciplinary staff.
Attend training in cultural competency, trauma-informed care, LGBTQ+ allyship, and best practices in homeless outreach and engagement.
Perform other job-related duties as assigned as needed

Qualifications:

Associate’s degree or relevant experience required
At least 1 year of experience working with homeless or unstably housed populations, preferably in astreet outreach, drop-in, or shelter setting.
Demonstrated understanding of mental illness, substance use, harm reduction, and trauma-informedcare.
Familiarity with NYC homeless services, 2010e, and PATH/CoC systems.
Excellent interpersonal and communication skills.
Ability to work independently and in a team.
Crisis de-escalation and motivational interviewing skills.
Strong organizational skills and attention to details.

Pay: $21.90 per hour
