Full Job Description

The Case Planner provides case management and casework services to children in foster care and their families. This role is responsible for assessing and monitoring child safety and well-being, while developing and implementing service plans to support timely permanency outcomes. The Case Planner works collaboratively with children, families, foster parents, service providers, and the courts to ensure coordinated care. Responsibilities include maintaining accurate documentation and ensuring compliance with all applicable regulatory and agency requirements.

Duties and Responsibilities:

· Manage newly assigned cases by completing timely intake activities, initial contacts, assessments, documentation, and service coordination.

· Conduct ongoing assessments of child safety, well-being, placement stability, service needs, and permanency barriers.

· Complete Permanency Hearing reports and court reports 30 days prior to the due date and submit them to supervisor

· Supervise Parent and child visits at the agency and in the community

· Complete required face-to-face contacts, foster home visits, birth parents, Trial discharge home assessments as required by ACS/OCFS

· Facilitate family engagement and support parent participation in case planning, visitation, court involvement, and service compliance.

· Attend all court hearing and knowledgeable of the cases

· Coordinate and monitor referrals to community-based, clinical, educational, behavioral health, and supportive services to address identified family needs.

· Work collaboratively with Birth Parent Advocates, Socio-Therapists, foster parents, ACS, legal representatives, service providers, and other multidisciplinary team members to support positive case outcomes.

· Ensure parents receive timely educational updates regarding their children in collaboration with the Education Unit.

· Monitor concurrent planning activities and ASFA requirements to support timely permanency outcomes, including reunification, kinship placement, guardianship, or adoption.

· Ensure internal and external parties are aware of cases changes within 48 hours

· Documentation is imputed in connection with 48 hours of the event.

· Attend and participate in TTM meetings, Family Team Conferences (FTCs), court hearings, transition planning meetings, and other required case-related conferences.

· Identify and report safety concerns, critical incidents, placement disruptions, compliance concerns, and permanency barriers in a timely manner.

· Perform other related duties as assigned by the Supervisor or designee.

Education & Experience:

· Must be fluent in reading, writing and communicating in both English and Spanish.

· Bachelor's degree required, with at least one (1) year of relevant child welfare, foster care, social services, or family-serving experience, OR Master’s degree in Social Work (MSW) or related Human Services field preferred

· Valid NYS Driver’s License preferred; ability to travel independently as required.

Pay: $58,000.00 - $62,000.00 per year
