Patient Care Coordinator

Liberty Hearing Centers | Multiple Brooklyn Locations

Position Summary

Liberty Hearing Centers is seeking a mature, compassionate, highly organized, and service-oriented Patient Care Coordinator to serve as the first point of contact for our patients, families, and referring providers. Key ResponsibilitiesPatient Experience & Customer Service

Greet patients, family members, and visitors with warmth, professionalism, and compassion.
Create a welcoming and supportive environment for patients throughout their hearing healthcare journey.
Communicate clearly and patiently with individuals who may have hearing loss, communication challenges, language barriers, or concerns about treatment.
Answer incoming phone calls, respond to inquiries, and assist patients with scheduling and follow-up needs.
Manage patient check-in and check-out processes efficiently and accurately.
Address patient concerns professionally and escalate issues when appropriate.

Scheduling & Office Coordination

Manage provider schedules to maximize patient access and clinic efficiency.
Confirm appointments and maintain schedule accuracy.
Help reduce cancellations and no-shows through proactive patient outreach and follow-up.
Support operations across multiple office locations when needed.
Ensure patient records, demographics, and documentation remain accurate and up to date.
Assist with administrative and operational tasks that contribute to the success of the location.

Insurance & Documentation Coordination

Collect accurate demographic, insurance, referral, and provider information from patients.
Ensure all required documentation is obtained and submitted promptly to the appropriate internal teams.
Work closely with Verification of Benefits (VOB), billing, clinical, and leadership teams to support a seamless patient experience.
Monitor outstanding referrals, prescriptions, physician orders, and required paperwork to help prevent delays in patient care.
Adapt to evolving payer requirements, documentation standards, and operational processes.
Understand that accuracy and attention to detail directly impact patient care, insurance processing, billing outcomes, and overall clinic success.

Referral Development & Community Engagement

Build and maintain positive relationships with referring physician offices and community partners.
Support outreach initiatives that strengthen referral relationships and increase awareness of our services.
Assist with community events, patient engagement activities, and practice growth initiatives.
Help support location goals through exceptional service, patient retention, and relationship building.

Team Collaboration

Work closely with audiologists, physicians, patient care coordinators, billing teams, verification teams, and leadership.
Communicate effectively across departments to ensure patients receive outstanding care and service.
Identify opportunities for improvement and actively participate in implementing solutions.
Embrace process changes and new initiatives with a positive and flexible attitude.
Foster a respectful, supportive, and team-oriented workplace culture.

QualificationsRequired

High School Diploma or equivalent.
Minimum of 2 years of customer service, healthcare, medical office, hospitality, patient-facing, or related experience.
Strong verbal and written communication skills.
Excellent organizational and time-management skills.
Proficiency with computers, scheduling systems, and standard office software.
Ability to multitask while maintaining accuracy and attention to detail.

Preferred

Experience in a healthcare, audiology, ENT, medical office, or customer service environment.
Experience working with seniors or medically diverse populations.
Experience coordinating referrals, scheduling, medical records, or healthcare documentation.
Bilingual or multilingual candidates are strongly encouraged to apply.
