Full Job Description

Compensation Range: $23/hr - $26/hr commensurate with experience

Benefits:

Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. Overview:

The Medical Case Manager (MCM) will provide all aspects of care coordination for the AIDS Institute (AI) Advancing Access Case Management clients: complete initial enrollment (all required AI and program forms/assessments), develop care plans, promote compliance with appointments and treatment regimens, and encourage self-sufficiency. The MCM will assist clients to access services, such as primary care, substance use treatment, mental health counseling, transportation, intensive care management, housing, and vocational/educational resources, through phone calls to clients and providers, and home visits when needed. In this role, the MCM will serve as a community-based advocate when meeting the client needs. The MCM will also participate in outreach events as needed.

Responsibilities:

Provide supportive case management to AI clients, with a focus on providing access to and support for those who are out of care and/or have an unsuppressed viral load.
Provide care coordination on an ongoing basis with other service providers, and participate in medical case conferences to promote integrated care. 
Serve as the primary point of contact for clients, ensuring seamless coordination and communication between healthcare providers, social services, and community organizations.
Assist clients in accessing and adhering to medical treatment, including antiretroviral therapy (ART), laboratory tests, and specialist consultations.
Follow up with the client and the referral provider to ensure that appointments are kept and that the patient is re-engaged in care and referred to needed treatment/prevention.  
Conduct Hep C testing as needed
Perform various administrative duties, including updating the client list and outreach logs; assisting with outreach calls, and scheduling intakes and follow-ups.  
Maintain essential data for reporting and program evaluation in eICare, including program enrollment, progress notes, care plans, referral activity, and updating contact information.  
Complete UND enrollments and coordinate referrals for mental health services, substance abuse treatment, housing assistance, legal aid, and other support services.
Provide comprehensive education on HIV/AIDS, treatment options, prevention strategies, and healthy lifestyle choices.
Promote sexual health and wellness through the use of risk-neutral approaches and evidence-based interventions.
Build and maintain resource lists for external services and emergency services (i.e., housing, domestic violence, food pantry) 
Participate in scheduled supervisory sessions with the Assistant Program Director and other Departmental Leaders as needed
Participation in youth community events, such as kiki balls, LGBT events, testing events, etc. 
Attend IDT meetings and daily huddles and complete updated info by due date in preparation for meeting.
Facilitate groups and EBIs and complete documentation within 48 hours.
Document all client interactions, interventions, and progress notes in accordance with program guidelines.

Other functions that may be required with this position include:

Participate in recurring interagency meetings, as well as external community meetings, to improve integrated care and maintain relationships with community partners  
Participate in department activities such as new staff onboarding, YPS Pride Committee, CQI efforts, etc 
Participate in Housing Works advocacy events (minimum 3 in person per year, 4 via other mode July-June) 
Perform other duties as assigned

Requirements:

Bachelor’s Degree required, plus 2 years of case management experience preferred
