Assistant Director of Rehabilitation
Position Overview
Responsible for directing and developing the Rehabilitation Services department to ensure the delivery of high quality rehabilitation care and services in accordance with all laws, regulations and facility guidelines.
Basic Qualifications
Education & Certification:
Graduate of an accredited PT, OT or Speech Therapy program (BS or MS degree); current licensed PT, OT or SLP within the state of operation; must be certified through the national association for specific discipline.
Skills & Requirements:
Must possess the ability to read, write and speak the English language, communicate resident condition to others and make independent decisions as needed; must have common working knowledge of all rules, laws and regulations as defined by the state as it relates to all therapy disciplines; must be patient, friendly, courteous and respectful to residents in any situation, including difficult ones; must have ability to implement rehabilitation programs, goals and objectives needed in the rehabilitation department; must be able to move frequently throughout the workday; must be able to handle sensitive situations related to the emotional well-being of the residents with patience, dignity and respect; must be able to lift 40 lbs floor to waist; lift 10 lbs waist to shoulder; lift and carry 40 lbs; and push/pull 40 lbs.
Experience:
Minimum of two-three years rehabilitation experience within a long-term care setting; experience in a rehab management role required.
Working Conditions
Works specifically in the rehabilitation department, resident rooms, etc.; bends, lifts, sits, stands and moves frequently throughout the shift; deals with frequent interruptions and possibly upset residents and family members; works regularly scheduled shifts with possible weekends, holidays and other hours based on resident needs; primary responsibility is care of residents but also deals with visitors, government personnel, etc.; participates in required continuing education programs; subject to odors, falls, burns and exposure to infectious diseases, waste, etc., including AIDS and Hepatitis B viruses.

Essential Job Duties
1. Directs and develops the rehab department to ensure the delivery of high quality care and services in accordance with all laws, regulations and facility guidelines.
2. Recruits, selects and orients qualified team members for the Rehabilitation Services Department.
3. Provides expectations and monitors performance of the Rehab department staff.
4. Responsible for overall supervision of the Rehab department and providing guidance to both management and non-management rehab team in accordance with budgetary obligations.
5. Schedules, attends and actively participates in care plan meetings as required by the facility guidelines.
6. Coordinates, schedules and actively participates in resident care conferences.
7. Provides verbal and written reports to the Administrator and makes care recommendations.
8. Assesses rehab treatment needs and accepts recommendations from the Rehab disciplinary team regarding the rehab department’s ability to meeting those needs and implements change as needed.
9. Assesses newly admitted residents and compares physician orders to care plans.
10. Monitors progress notes to ensure information recorded is timely, accurate and descriptive.
11. Monitors treatments provided by the rehab department to ensure therapy needs have been met as outlined on the care plans.
12. Investigates grievances or complaints regarding the rehab department in a timely manner to determine the best resolution and forwards to the Administrator.
13. Monitors licenses and certifications for rehab team to ensure all are valid and current.
14. Ensures guideline and procedure adherence and monitors absenteeism; meets with team members as needed to perform necessary corrective action steps.
15. Provides information for denials and other intermediary requests for documentation.
16. Identifies opportunities for rehab services as it relates to outpatient therapy and ensures delivery.
17. Ensures timely completion of MDS coding and care plan objectives by reviewing necessary reports.
18. Creates and monitors the Quality Assurance program, reviews reports and makes recommendations to the QA improvement team.
19. Ensures appropriate staffing of PT, OT, SLP and RT disciplines in accordance with local and state guidelines.
20. Ensures that the rehab services team is compliant in following all infection control measures as provided by the nursing department.
21. May be required to participate in various routine and non-routine meetings to enhance the effectiveness and efficiency of facility outcomes.
22. May participate in performing investigations in accordance with state guidelines for allegations of resident neglect, abuse or misappropriation of resident property.
23. Oversees the assessment of functional levels and capabilities of residents.
24. Conducts ongoing assessments of resident muscle strength, range of motion, coordination and balance and makes recommendations related to resident therapy treatments.
25. Assists in providing therapy treatments by following the resident plan of care.
26. Develops goals which are measurable and realistic, based on resident’s care plan.
27. Thoroughly documents treatment progress in resident charts in an accurate and timely manner.
28. Provides training and education to family, residents and staff for rehabilitation services and use of assistance devices and equipment.
29. Oversees the home evaluation process prior to discharge to ensure safety of residents.
30. Maintains a positive working relationship with residents and families.
31. Completes required facility forms and documents to ensure compliance with state and federal regulations.
Other Job Duties
32. Assists with transporting residents from room to therapy gym, etc.
33. Schedules appointments, takes calls, reorders supplies and completes insurance forms.
34. Assists in the admission process for selection of new residents.
35. Actively participates in development and delivery of in-service training programs.
36. Participates in facility and department orientation program for new employees.
37. Assists with administrative duties as needed or requested.
38. Communicates new admissions with rehab staff.
39. Provides direct rehab care as necessary.
40. Promotes continuity of care through partnering with other departments (i.e. Nursing).
41. Participates in discharge planning process.
42. Actively participates in state or federal survey processes.
43. Educates staff on facility policies regarding resident right to refuse treatments.
