Full Job Description

Program Function: Under the supervision of the Program Director, the Street Housing Navigator is responsible for management of the day-to-day operation of the Coastal Coordinated Entry Navigation Program. Including the provision of services and CCESNP staff in accordance with the agency’s stated purpose and in such a manner that optimum results are achieved in relation to the resources of the agency.

General Function / Job Requirements

The Housing Navigator is responsible for providing intensive case management, housing navigation, staff support, and program oversight services to individuals and families experiencing homelessness or housing instability. Services begin with client intake, assessment, eligibility verification, and coordinated entry participation to ensure participants are connected to appropriate housing and supportive services.

The Housing Navigator will ensure all participants are assessed utilizing the Coordinated Entry framework and that all services, case notes, referrals, and outcomes are accurately documented within the Homeless Management Information System (HMIS) in accordance with agency, state, and federal requirements. This position is responsible for maintaining compliance, data quality, confidentiality, and accountability standards always.

The Housing Navigator will develop individualized housing stability plans focused on securing and maintaining permanent housing outcomes. Ongoing support services will include case management, crisis intervention, service coordination, referrals to community resources, and regular monitoring of participant progress toward established goals.

This position will also provide support and oversight to program staff as needed, including assisting with program development, staff training, goal implementation, and ensuring quality service delivery. The Housing Navigator will ensure staff maintain complete, accurate, and confidential client records in compliance with program guidelines and funding requirements.

Housing navigation responsibilities include identifying, mapping, and maintaining available housing resources and rental units throughout the Coastal Georgia region in coordination with the South Georgia Coordinated Entry System. The Housing Navigator will participate in weekly case review meetings with Coordinated Entry lead case managers and community partners to support collaborative service planning and housing placement efforts.

The Housing Navigator will assist participants with all aspects of the housing search and lease-up process, including:

· Completing housing applications

· Securing identification and required documentation

· Coordinating transportation to housing appointments and unit viewings

· Communicating with landlords and property managers

· Assisting with lease reviews and move-in coordination

· Supporting positive tenant-landlord relationships

The program will actively engage landlords and property managers to expand access to safe, stable, and affordable housing opportunities across the service region. Follow-up and after-care services will be provided to promote long-term housing stability and reduce the likelihood of returns to homelessness.

Supportive Services and Client Engagement

Participants will receive access to supportive services designed to increase self-sufficiency, independence, and long-term stability. Services may include:

· Life skills training

· Independent living support

· Individualized goal planning

· Education and employment referrals

· Community resource linkage and advocacy

Youth participants will receive additional support connected to educational opportunities, workforce development, and community-based services. Program participants may also receive or be connected to:

· GED preparation

· ESL and basic education services

· Consumer and health education

· Job readiness training

· Employment search assistance

· Workforce development and vocational training programs

As allowed under grant and program guidelines, participants may receive supportive assistance such as transportation support, work clothing, certification costs, training fees, and other barrier-reduction services that promote employment and housing stability.

Program Coordination and Community Engagement

The Housing Navigator will actively participate in coordinated entry meetings, treatment team meetings, community partner collaborations, and regional planning activities to support a coordinated and comprehensive system of care.

Staff are required to complete a minimum of 10 hours of annual professional development and training to ensure continued compliance and high-quality service delivery.

The Housing Navigator will also support outreach and community engagement efforts by maintaining strong working relationships with:

· Schools and educational partners

· Child welfare agencies

· Landlords and property managers

· Workforce and employment agencies

· Healthcare and behavioral health providers

· Community and social service organizations

This position requires strong communication, organizational, advocacy, and problem-solving skills, along with the ability to work effectively with diverse populations experiencing homelessness, housing instability, and other barriers to self-sufficiency.

Education & Related Work Experience: (minimum& preferred educational requirements necessary to perform this job successfully)

· High School Diploma is required.

· Associate or bachelor’s degree preferred.

· Background in Housing and landlord engagement

· Related Experience working with target population (Preferred)

· Must be 26 years of age or older.

· Willingness to work some weekends and evenings.

· Valid driver's license and good driving record.

· Must be able to reflect on one’s internal state utilizing appropriate resources.

· Average Hours per week 40

· Must be able to pass a drug screen and background check.

Skills & Knowledge

Required:

· Knowledge of community resources and experience in providing referral services to clients.

Pay Range $ 40,000 to 50,000
