Overview:

FT/40 hrs. This role is responsible for coordinating the care of assigned clients by working collaboratively with both internal and external service providers. The ideal candidate will be skilled in connecting clients and their families to a wide range of supportive services that promote overall well-being and stability. This position also involves advocating for clients and empowering them to access the resources they need to thrive. Bachelor’s degree preferred. Qualified candidates must have experience working with clients in one or more populations of focus, either in an employment or volunteer capacity. Fluency in Spanish is required. Starting pay rate of $28.00 - $28.50 per hour based on bilingual differential.

Responsibilities:

Engage clients and families, explain program goals, and complete intake assessments.
Involve clients and families in goal-setting and planning.
Develop individualized plans reflecting client needs and cultural values.
Connect clients to community resources and strength-based services.
Advocate for clients and act as liaison with service providers.
Maintain communication and professional boundaries.
Meet clients in various locations as needed.
Complete documentation accurately and on time.
Participate in training, supervision, and quality improvement activities.

Qualifications:

Bachelor’s degree in human services or a related field requiered.
Experience working with clients in one or more population of focus, in an employment or volunteer capacity.
Must maintain eligibility to work under federal and/or state mental health funded programs.
Valid California Driver’s License and acceptable driving record in accordance with Olive Crest’s insurance carrier.
Must obtain and maintain criminal record clearance.
Working knowledge of computer programs including Outlook and Word. Trainable in use of Electronic Health Record.
Employee must have a philosophy of support for at-risk children and families in line with the mission, vision, values, and statement of faith of Olive Crest.

Up to 48 hours per year for Bereavement with death in immediate family
