What You'll Do

Complete hospice admissions, eligibility evaluations, informed consents, education, and initial care planning.

Provide case management support as needed, including symptom management, medication oversight, and ongoing care coordination.

Complete routine, PRN, urgent, and crisis field visits throughout the Twin Cities metro area.

Collaborate closely with physicians, nurse practitioners, social workers, chaplains, hospice aides, and caregivers to provide exceptional interdisciplinary care.

Educate patients and families on disease progression, comfort-focused care, and hospice services.

Maintain timely, accurate EMR documentation while ensuring compliance with Medicare Conditions of Participation and hospice regulations.

Adapt quickly to changing clinical priorities while delivering compassionate, patient-centered care.
What We're Looking For

Active Minnesota Registered Nurse (RN) license.
-
Hospice experience preferred, or strong clinical experience in ICU, oncology, palliative care, home health, emergency medicine, or other complex care settings.

Excellent assessment skills, critical thinking, and independent clinical judgment.

Comfortable managing changing priorities while maintaining exceptional patient care.

Outstanding communication and relationship-building skills with patients, families, and interdisciplinary teams.

Reliable transportation and willingness to travel throughout our service area.

A compassionate nurse who enjoys learning, problem-solving, and making a difference wherever needed.
