Assistant Director of Rehabilitation Services

We’re looking for an Assistant Director of Rehabilitation Services who’s ready to bring energy, leadership, and strong clinical skills to a team that truly cares about what they do.

This is a hands-on leadership role where no two days look exactly the same — you’ll treat patients, support the rehab team, and step into leadership coverage when the Director is out. If you like a role where you can stay clinically sharp and grow your leadership impact, this is it.

What You’ll Do

You’ll be part clinician, part leader, part problem-solver — and a key driver of our rehab department’s success.

Deliver high-quality, skilled therapy treatments that help residents regain independence
Support day-to-day rehab operations and help keep everything running smoothly
Work closely with an interdisciplinary team to ensure strong, coordinated patient care
Maintain excellent clinical standards, documentation, and compliance
Mentor and support therapy staff — be the go-to resource when questions come up
Step into the Director of Rehabilitation Services role when needed (you’ve got backup, but also trust)
Help create a rehab culture that’s positive, collaborative, and patient-first

Responsibilities:

Assistant Director of Rehabilition Services:

Assumes the duties of the Director of Rehabilitation Services in absence of the Director.
Initiates skilled therapy treatments in accordance with physician’s orders and rehabilitation goals; maintains required contract with physician.
Maintains written evaluations, progress notes, MDS records and discharge summaries for residents in therapy program according to state, federal and facility policies; makes written recommendations for inter-disciplinary care planning and treatment.
Collaborates with other members of the inter-disciplinary health care team to integrate rehabilitation goals into the comprehensive care plan.
Provides input for Resident Care Planning (RCP) and Resident/Family Care Plan Review meetings; attends when appropriate; completes required documentation after the meetings.
Maintains statistical records of residents’ treatments.
Assists with education and in-service trainings when needed.
Records and updates individuals’ care plan(s).
Informs individual or qualified representative about care and treatment plan and possible outcomes of treatment, or lack of treatment if resident refuses participation in a therapy program.
Orients, trains and supervises departmental personnel; assists with students assigned to the facility or educational programs.
Compiles and submits requisitions for supplies and equipment.
Acts as direct report for Therapy/Rehab personnel in conjunction with Director.
Ensures all documentation is processed accurately and timely.
Leads departmental meetings (in Director’s absence) and attends the following inter-departmental meetings when appropriate:
a. Care Planning
b. Quality Assurance Committee
c. Quality Assessment and Review Committee
d. Morning Report
e. Safety Committee
f. In-service Education
g. Staff Meeting for Department heads and Supervisors
Arrives to work on time, regularly, and working as scheduled; responsible for own transportation.
Utilizes electronic timekeeping system, if required.
Performs assigned duties according to the service excellence standards and goals established for this position, and for all staff members of this facility.

Qualifications:

Assistant Director of Rehabilition Services:

Licensed Occupational Therapist, Physical Therapist, or Speech Language Pathologist professionally educated in an accredited curriculum with a baccalaureate degree
License to practice in the state in which work is performed
Must have interest in working with the elderly and respect the rights of residents during care and treatment services
Displays genuine concern for the elderly and patient and compassionate attitude
Current, valid CPR/BLS certification
Possession of ability to teach, inspire and motivate others; ability to organize workload, delegate assignments and promote teamwork required
Experience in the geriatric field of Occupational/Physical Therapy or
Speech Language Pathology preferred
Experience with computer hardware and software including
Microsoft Office preferred
This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
