Full Job Description

The Registered Nurse Transfer Center is responsible for evaluating, coordinating, processing, screening, and documentation of patient transfers into the CHRISTUS Health System. The CHRISTUS Transfer Center RN will collaborate with relevant business partners for appropriate level of care and transport of patients into the CHRISTUS Health system.

The CHRISTUS Transfer Center will be responsible for obtaining appropriate medical and demographic information and coordinating communication between the referring and receiving physician as well as between healthcare facilities.

The CHRISTUS Transfer Center RN will efficiently engage appropriate departments in the transfer process. The CHRISTUS Transfer Center RN will utilize problem solving and customer service skills to determine the best course of action for the patient, the physician, and the hospital by working closely with facility House Supervisors, referring physicians, ED and inpatient staff to ensure the effective and efficient transfer of every patient.

Requirements:

Associate's Degree in Nursing
RN License in state of employment or compact
BLS
3+ years acute care preferred
5+ years in ED, ICU, or critical care is especially valuable

Exhibits excellent customer service skills, including telephone and personal contact with all CHRISTUS Transfer Center groups (i.e. physicians, case managers, nurses, administrators, and colleagues)

Evaluate and process transfer requests from outside hospitals, clinics, and healthcare facilities seeking to transfer patients into CHRISTUS Health facilities.
Coordinate patient transfers by gathering medical and demographic information and ensuring all required documentation is completed accurately.
Facilitate physician-to-physician communication between referring providers and r
Work with House Supervisors, Emergency Departments, inpatient units, and administrators to determine bed availability, patient placement, and appropriate level
Arrange appropriate patient transportation and ensure the transfer process is coordinated from start to finish.
Monitor patient flow and hospital capacity by collaborating closely with hospital leadership and nursing supervisors.
Ensure EMTALA compliance and adherence to transfer policies and procedures.
Prioritize urgent and critical cases, identify barriers to transfer, and escalate issues when needed.
Provide exceptional customer service to physicians, nurses, case managers, administrators, patients, and families involved in the transfer process.

Pay: $29.00 - $38.00 per hour
