Full Job Description

Qualified Intellectual Disability Professional (QIDP) position will manage a caseload of individuals who are developmentally disabled. This person must be qualified according to DHS guidelines. Bachelor’s degree in Human services field. Minimum of 4 years’ experience working with individuals with developmental disabilities. Must be Qualified Intellectual Disabilities Professional (QIDP) certified Minimum 4 years supervisory experience. This position supervises House Managers to ensure that the daily living of residents meets program and quality outcomes.

RESPONSIBILITIES:

Create, schedule and implement ISP (individual service plan), goals/objectives, and the like. Reviews and revises when necessary the Individual Support Plan through the development of new goals and objectives reflective of the individuals current needs.

Makes sure delivery of Residential services are in compliance with DHS Rule 50, 115, 116 and 119, DD waiver standards, agency mission, agency policy & procedure, program guidelines and best practices. Keeps up to date with all applicable laws, rules/regulations, and policies and procedures published through agencies responsible for surveillance and regulatory oversight of residential services.

Provides direct supervision to DSP staff, completes performance appraisals and initiates disciplinary action when necessary. Conducts staff training in program implementation in all goal areas. Trains new DSP’S or conducts in-services for current DSP’S using necessary materials in accordance to DHS guidelines

Monitors budgets for the residential program, including expenditures, and general compliance with budgetary and fiscal practices including program participant financial records.

Provide case management services for clients, including social security compliance rules, Medicaid, annual physicals, dental and vision, overseeing financials of individuals.

Convenes and chairs the IDT meeting annually and more often as necessary.

Conducts annual assessments of each individual using standardized assessment tools including the ICAP and others required by policies and procedures.

Interact with Guardians to ensure client satisfaction. Advocates, promotes, and protects the rights of all individuals.

Reviews Unusual Incident Reports and other documentation relating behavioral and health status of each individual and any trends, which would require further evaluation and possible corrective action.

Responds to medical and/or behavioral crisis situations immediately and provides support to staff as needed. Completes and provides written follow-up on Unusual Incident Reports as necessary.

Be a liaison between all parties involved with residential care. Example: Day Training facilities, Production Groups, Behavior Management Teams, etc.

Ensures all record keeping/documentation is complete, accurate, and up to date as required.

Responsibilities include, but are not limited to: monthly progress notes, tracking sheets, sending/retrieving required documentation to/from guardians in a timely manner, etc.

Ensures compliance with established safety procedures and other regulations.

Oversees the home and works with House Manager(s) to ensure adequate coverage and appropriate activities are consistent within the program.

REQUIREMEMTS, EDUCATION & EXPERIENCE:

Provides 24 hour emergency coverage as needed.

Maintains American Heart Association certification in First Aid and CPR.

Must be proficient in the use of computers and softwares like Microsoft Word and Excel. Valid Illinois drivers license with clean driving record.

Bachelor’s degree in Human services field. Minimum of 4 years’ experience working with individuals with developmental disabilities.

Must be Qualified Intellectual Disabilities Professional (QIDP) certified with a minimum 4 years supervisory experience.

Pay: $25.00 - $27.00 per hour
