This role operates in a Day Habilitation (DAY HAB) Setting, defined as services “that provide opportunities and support for community inclusion and build interest in and develop skills for active participation in recreation, volunteerism and integrated community employment.” (Iowa Health & Human Services). Day Habilitation provides assistance with acquisition, retention, or improvement of socialization, community participation, and daily living skills.

Person-Centered Community Engagement: Collaborate with individuals to identify interests, strengths, and goals, and develop meaningful daily and weekly schedules that include community participation, hobbies, volunteer work, and cultural, civic, or religious activities.

Goal-Based Skill Development: Provide direct support by teaching individuals how to complete tasks and achieve goals independently, using coaching and instruction rather than doing tasks for them.

Behavioral Support Implementation: Follow individualized behavioral care plans by applying approved strategies, encouraging participation, motivating progress, and supporting positive behavior in alignment with each person’s goals.

Documentation & Progress Tracking: Maintain accurate, consistent, and timely documentation of individual progress, activities, and outcomes to ensure continuity of care and accountability.

Safety & Advocacy: Provide a safe, supportive environment while respecting individual choice and autonomy, advocating for independence, dignity, and person-directed decision-making in all aspects of service delivery.

Driver’s License: You’ll be responsible for transportation at times, so we need to trust your ability to be safe behind the wheel.

Reliable Transportation: Because you’ll be responsible for helping connect individuals to employment opportunities, you’ll need reliable transportation. You will need to provide proof that your automobile insurance covers $100,000/$300,000/$50,000 liability coverage – don’t worry, we can answer any questions you might have about this.

Education Requirement: While no higher education is necessary, we do look for candidates who have obtained their diploma or the equivalent representing their high school education.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
