Locations: New Mexico (NM), New Hampshire (NH), Vermont (VT), Wisconsin (WI), Maine (ME) and vermont (VT)

Contract: 8–13 weeks (extendable)
Guaranteed Hours: 36 - 40 hours/week
Hourly Pay Rate: $32–$48/hr

Job Summary
We are seeking a dedicated and versatile Licensed Practical Nurse (LPN) for travel assignments to join our healthcare team. The ideal candidate will provide high-quality nursing care across a variety of clinical settings, including hospitals, long-term care facilities, assisted living communities, and home health environments. This role offers an exciting opportunity to work in diverse healthcare settings, delivering essential patient care while gaining valuable experience in specialized areas such as critical care, geriatrics, and emergency medicine. The LPN will be responsible for assessing patient needs, administering treatments, and ensuring compliance with healthcare regulations and confidentiality policies.
Responsibilities
- Deliver direct nursing care to patients in LTC, SNF, rehab, and assisted living settings
- Perform patient assessments and report changes in condition to RN or physician
- Administer medications and treatments per provider orders and facility policy
- Document all patient care, observations, and interventions in electronic health records (EHR)
- Collaborate with interdisciplinary teams including physicians, RNs, therapists, and case managers
- Educate patients and families on care plans, medications, and discharge instructions
Skills
- Active LPN/LVN license in applicable U.S. state (or compact eligibility where accepted)
- Current BLS/CPR certification
- Ability to work in LTC, SNF, or similar clinical environments
- Strong communication, documentation, and clinical decision-making skills
- 1+ year of experience in Long-Term Care, Skilled Nursing Facility, Rehab, or Home Health
- Experience with EHR systems (Epic, Cerner, Meditech, or similar)
- Prior travel or contract nursing experience
- Ability to quickly adapt to new facility workflows and policies
Shift & Schedule
- 12 hrs shifts Days/ Night/Evening
- Weekly Hours: 36hours (guaranteed)
- Schedule :12x3, 8x4
- Call Requirements: Typically, not required (facility dependent)
Compensation & Benefits
- Pay Range: $32 – $48 per hour (based on experience, location, and assignment type)
- Travel Stipend: Available for eligible travel assignments (clinicians traveling more than 50 miles from their tax home location, subject to verification and compliance rules)

Work Setting & Physical Requirements
- Settings: Long-Term Care (LTC), Skilled Nursing Facilities (SNF), Rehabilitation Centers, Assisted Living, Home Health
- Ability to lift, push, or pull up to 35–50 lbs
- Prolonged standing and walking during shifts
- Required use of PPE and adherence to infection control protocols
Compliance & Credentialing
- Background check and drug screening required
- Immunizations required per facility policy (e.g., MMR, Varicella, Tdap, Flu, COVID-19 where applicable)
- TB screening and fit testing as required
- Active license and certifications must be valid prior to start date
