Job Overview

We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Caregiver to join our team in providing exceptional patient-centered care. In this vital role, you will support individuals across various care settings, including assisted living, nursing homes, and home healthcare environments. Your energy and attention to detail will help enhance the quality of life for our clients by assisting with daily activities, monitoring health status, and ensuring their comfort and safety. This position offers an opportunity to make a meaningful difference in the lives of those you serve while working within a supportive, team-oriented environment.

Responsibilities

Provide personal care assistance including bathing, dressing, grooming, and mobility support
Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and transferring using patient transfer methods and equipment like Hoyer lifts
Monitor and record vital signs such as blood pressure, pulse, respiration, and temperature accurately in electronic health record (EHR) or electronic medical record (EMR) systems
Administer medications following home healthcare regulations and physician instructions while maintaining strict patient confidentiality in compliance with HIPAA standards
Observe and report changes in patient condition, including symptoms related to dementia, Alzheimer's disease, or other memory care needs
Support patients with end-of-life or hospice care with compassion and dignity
Maintain a clean environment by assisting with laundry, cleaning, meal preparation, and ensuring safety protocols are followed during heavy lifting or physical tasks
Document medical records thoroughly and accurately to ensure seamless communication among healthcare providers
Assist with medical procedures such as catheter care or wound management when trained and qualified to do so
Communicate effectively with patients, families, and healthcare teams to ensure personalized care plans are followed

Experience

Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies preferred
Demonstrated knowledge of medical terminology and basic life support (BLS) or CPR certification required
Experience working with individuals with developmental disabilities or dementia/dementia care is highly valued
Familiarity with EMR/EHR systems for documentation and patient records management is advantageous
Proven ability to perform patient transfers safely using techniques such as Hoyer lifts or other transfer methods
Experience providing long-term care or live-in employment is a plus
Knowledge of home & community care regulations and best practices for geriatric patient care is essential
