Inpatient Pharmacist Opportunity

A respected California healthcare organization is seeking a dedicated Inpatient Pharmacist to join its hospital pharmacy team. This is an excellent opportunity for a clinically focused Inpatient Pharmacist who is passionate about medication safety, interdisciplinary collaboration, and improving patient outcomes in an acute care setting.

As an Inpatient Pharmacist, you will play a critical role in ensuring the safe and effective use of medications throughout the hospital. Working closely with physicians, nurses, respiratory therapists, and other healthcare professionals, you will provide clinical pharmacy services, monitor medication therapies, and support high-quality patient care.

Whether you are an experienced Clinical Pharmacist, Hospital Pharmacist, or an Acute Care Pharmacist looking for your next opportunity, this position offers the chance to make a meaningful impact while working within a collaborative healthcare environment.

Key Responsibilities

Clinical Pharmacy Services

Review, verify, and process medication orders to ensure accuracy and appropriateness.
Prepare and dispense medications in accordance with established pharmacy standards.
Monitor patient medication therapies and recommend interventions when necessary.
Identify, prevent, and resolve medication-related problems.
Participate in medication reconciliation and therapeutic drug monitoring activities.

Patient Care

Provide direct pharmaceutical care to hospitalized patients.
Evaluate medication regimens for safety, efficacy, and cost-effectiveness.
Educate patients and healthcare providers regarding medication therapy.
Support evidence-based medication management and clinical decision-making.

Interdisciplinary Collaboration

Collaborate with physicians, nurses, respiratory therapists, and other healthcare professionals.
Participate in patient care rounds and clinical consultations when appropriate.
Provide drug information and recommendations to optimize patient outcomes.
Assist in developing medication protocols and best practices.

Inventory & Medication Management

Maintain accurate medication inventory levels and ordering systems.
Ensure proper storage, handling, and security of medications.
Monitor medication utilization and assist with formulary management initiatives.
Support medication shortage management and alternative therapy recommendations.

Regulatory Compliance & Quality Assurance

Ensure compliance with all local, state, and federal pharmacy regulations.
Maintain adherence to HIPAA, DEA regulations, USP <797>, USP <800>, and applicable accreditation standards.
Participate in quality improvement and medication safety initiatives.
Assist with pharmacy audits, inspections, and policy updates.

Professional Development

Stay current on pharmacy best practices, emerging therapies, and regulatory changes.
Participate in continuing education and professional development activities.
Maintain all required licensure and certifications.

Qualifications

Required

Current and unrestricted California Pharmacist License.
Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD).
Strong knowledge of hospital pharmacy operations, medication safety, and regulatory requirements.
Familiarity with pharmacy guidelines, standards, and regulations.
Strong verbal and written communication skills.
Ability to thrive in a fast-paced acute care environment.
Strong organizational, critical thinking, and problem-solving skills.
Proficiency with pharmacy information systems and computer applications.

Preferred

PharmD degree.
Previous hospital or inpatient pharmacy experience.
Experience with electronic medical records (EMR) and computerized physician order entry (CPOE) systems.
Clinical pharmacy experience in an acute care setting.
