Full Job Description

Program Director – Adult Day Health Care / CBAS

We are seeking an experienced and compassionate Program Director to provide leadership and oversight of our Community-Based Adult Services (CBAS) program.

The Program Director is responsible for the overall implementation, coordination, and daily management of the program, ensuring that participant needs are met and that services are delivered in accordance with applicable licensing, regulatory, and organizational requirements. This is a full-time, on-site leadership position requiring strong knowledge of adult day health care, interdisciplinary care, staff supervision, documentation, and regulatory compliance.

Key Responsibilities

Provide overall leadership, coordination, and oversight of the CBAS program and its daily operations.
Maintain regulatory and survey readiness and assist with California Department of Aging (CDA) and California Department of Public Health (CDPH) regulatory visits, including documentation review, regulatory inquiries, and required follow-up and corrective actions as needed.
Continuously evaluate participants' changing needs and ensure appropriate adjustments are made to their services and plans of care.
Coordinate and chair the Multidisciplinary Team (MDT) process, including initial assessments, reassessments, eligibility reviews, and development and updating of participant care plans.
Review participant documentation for completeness, accuracy, and compliance with program requirements.
Provide final oversight and approval of participant plans of care prior to submission, as applicable.
Supervise employees, establish staff roles and responsibilities, monitor performance, and ensure adequate support is provided to meet participant needs.
Conduct and support staff performance evaluations, attendance monitoring, corrective action documentation, and sensitive employee discussions.
Provide and coordinate staff in-service education regarding licensing requirements, regulatory requirements, agency policies, participant care, and other required topics.
Maintain oversight of daily attendance, reassessment tracking, quarterly tracking requirements, meal/CACFP reporting, and other required program reports.
Monitor participant eligibility, billing-related corrections, and required program documentation.
Oversee contract staff and monitor the delivery and hours of contracted services.
Maintain compliance with applicable health and safety requirements, including licensing, fire safety, emergency/disaster preparedness, and organizational policies.
Serve as a liaison among participants, families, employees, administration, health plans, and other involved parties.
Maintain effective communication with administration regarding program operations, participant concerns, staffing matters, compliance issues, and other significant developments.
Oversee participant incident documentation and appropriate follow-up.
Promote a professional, collaborative, participant-centered environment throughout the Center.

Minimum Qualifications

In accordance with California Department of Aging requirements, the Program Director must:

Be a qualified professional in nursing, social work, psychology, recreational therapy, occupational therapy, or physical therapy.
Demonstrate competence, or potential competence, in working with impaired elderly individuals living at home.
Be knowledgeable of physical, social, and mental health programs operating within a licensed health facility or clinic.
Hold any applicable California professional license, registration, or certification required for their discipline.
Be able to serve as a full-time Program Director during the Center's hours of operation and remain on the premises and available to participants, participants' relatives, and employees.
Have no other duties during program hours except those related to adult day health care and not be included in the staffing ratios of another licensed facility.

Preferred Qualifications

Previous experience in an Adult Day Health Care (ADHC) or Community-Based Adult Services (CBAS) program.
Bilingual in English and Spanish strongly preferred to effectively communicate with participants, families, and staff.
Experience working with older adults and adults with functional, cognitive, physical, or behavioral health needs.
Experience coordinating or leading a multidisciplinary team.
Knowledge of participant assessments, reassessments, individualized plans of care, documentation, and regulatory compliance.
Previous supervisory or healthcare management experience.
Strong leadership, communication, organizational, and problem-solving skills.

Employment Requirements

The selected candidate must meet applicable employment and program requirements, including:

Current First Aid and CPR certification or completion within the required timeframe.
Health clearance demonstrating ability to perform the duties of the position.
Negative tuberculosis (TB) testing.
Department of Justice fingerprint/background clearance.

Salary Range: $60,000-$96,000 annually

Compensation within the range will be determined based on relevant experience, professional qualifications, licensure, and demonstrated experience in ADHC/CBAS or a related healthcare setting.

Pay: $60,000.00 - $96,000.00 per year
