This position is for a full-time Resident Health Services Coordinator position to work at Bella Terra and Cochrane Village, a 40-unit and 96-unit affordable housing property in Morgan Hill, CA. Qualified Candidates will have a Bachelor’s degree in social services, public health, or related field. 3 to 5 years’ experience in human services, community-based work, CHW roles, including at least two years’ experience with supportive housing or social services. Lived experience or deep familiarity with the communities served. Must have valid driver’s license and DMV clearance. Community Health Worker (CHW) certification or willingness to obtain certification preferred. Master’s degree in Social Work, Public Health, or related field preferred. Familiarity with Medi-Cal, CalAIM, MCPs, or county service systems a plus. Bilingual/multilingual, a plus. Salary range: $25.00 - $37.00 per hour; hiring range for new employees is generally $25.00 - $31.00 per hour, DOE. Please note an offer will take into consideration the experience of the final candidate and the current salary level of individuals working at EAH in a similar role.

POSITION OVERVIEW
Assists residents in achieving housing stability, improved health outcomes, and overall wellbeing. Engages residents with culturally responsive service navigation, health-related support, and community-building activities within EAH Housing communities. Connects residents to existing social services, examines needs for on-site services, and explores ways to bring programs to the property, serving as a bridge between residents, property management, service providers, Managed Care Plans (MCPs), and community-based organizations. This position is characterized by a high degree of initiative, accountability, and ability to work congenially with a wide variety of individuals within EAH and outside of the organization. Understands and demonstrates commitment to the Mission and Values of EAH Housing.
RESPONSIBILITIES
- Coordinates and provides non-clinical, health-related support, including health education and
promotion, navigation of healthcare, behavioral health, and social service systems.
- Assists residents in understanding and accessing Medi-Cal benefits, primary and specialty
care, behavioral health, and substance-use services.
- Supports identification and warm handoffs to CalAIM Community Supports or Enhanced Care
Management (ECM) as applicable, coordinate with MCPs, CHW supervisors, and service
partners as required.
- Supports residents in addressing social drivers of health, including food security,
transportation, benefits access, and housing stability.
- Connects residents to food resources in their community. Increases resident access to food
by facilitating distributions onsite as needed.
- Coordinates and oversees the delivery of other resident services and program activities to
residents both on and off-site. Provides information, assistance, and resident services
referrals and engages in follow-up. Performs needs assessments to determine what types of
programs and services would benefit the residents. Obtains resident feedback on
effectiveness and quality of programs and their delivery through an annual Resident
Satisfaction Survey. Develops supportive relationships with residents and conducts regular
outreach to encourage participation in resident programs.
- Provides and coordinates opportunities for residents to learn new skills, build community and
promote self-sufficiency, including social, educational and StayWell activities (e.g.,
celebrations, classes).
- Establish connections with service providers and maintain community resource directory in
online database.
- Creates monthly newsletters and calendar of events and activities.
- Maintains collaborative relationships with property managers and REM staff. Participates in
weekly meetings with the site team.
- Actively participates in company-wide special initiatives, such as scholarships and digital
learning, as applicable.
- Assists with recruiting and coordinating volunteers and vendors as needed.
- Assists residents in building informal support networks with residents, family, and friends.
- Attends all Resident Council meetings and works with the Council and residents to encourage higher levels of participation and access to program activities.
- Promotes resident leadership, peer support, and community connection; supports culturally relevant programming and contributes to trauma-informed, equity-centered service delivery.
- Interfaces regularly with provider agencies, public agencies, and community volunteer groups to maximize effectiveness of program activities. Monitors participating agency involvement for quality and compliance with memorandums of understanding or other service agreements.
- Maintain accurate, timely documentation of resident engagement, service coordination activities, and budgets using approved systems and tools.
- Adheres to confidentiality, HIPPA, fair housing, and organizational policies.
- Attends department and company meetings, trainings.
- Actively participates in EAH’s Injury and Illness Prevention Plan.
- Regular and predictable attendance.

QUALIFICATIONS
Bachelor’s degree in social services, public health, or related field. 3 to 5 years’ experience in human services, community-based work, CHW roles, including at least two years’ experience with supportive housing or social services. Lived experience or deep familiarity with the communities served.

- Must have access to reliable transportation. If using own vehicle, must maintain own auto insurance in accordance with EAH minimum requirements
DESIRABLE ADDITIONAL QUALIFICATIONS

- Master’s degree in Social Work, Public Health, or related field.
- Familiar with local, state, and federal housing regulations.
- Familiarity with funding sources for housing and programs.
- Familiarity with Medi-Cal, CalAIM, MCPs, or county service systems.
- Ability to manage multiple priorities, including complex and time sensitive projects.
- Bilingual/multilingual, a plus.
CRIMINAL BACKGROUND CHECK REQUIREMENTS
EAH Housing conducts criminal background checks on all candidates who have received a conditional job offer.
