Description

As a Hospice Client Care Coordinator (CCC), you will be called to care when you’re needed most. As part of Interim HealthCare, you’ll support a full range of patient services to bring comfort and dignity to our clients.

As a Hospice Client Care Coordinator (CCC), you will:

Facilitate new client enrollment with initial and subsequent screening assessments and further maintain client’s healthcare record
Identify resources to address client’s health, daily living, and safety needs
Perform administrative tasks such as scheduling, logging, documenting all appointments and services
Communicate to clients through telephone and computer
Coordinate with RN and pharmacy medication set-ups and reminders
Be a family liaison and coordinate with the family, designees, and providers

To qualify to be a Hospice Client Care Coordinator (CCC) with us, you will need:

Education: High School graduate or equivalent
One year of related healthcare experience is required
Experience in working in the senior field or hospice is preferred
Must have a valid driver’s license
