Full Job Description

Lead the Future of Ohio Youth as a Manager of OhioRISE Care Coordination/Case Management with Coleman Health Services.

Are you ready to shape the trajectory of Ohio's youth while being a part of a growth-oriented management team? Your passion for leading others and contributing to system change is what we are looking for! Join us as the Supervisor of OhioRISE Case Management/Care Coordination where you will lead a Care Coordination team that drives positive change in the lives of youth and families within our community. We are looking for a manager to be based out of Akron and serve Summit County, Ohio. Great opportunity for those with a background in social work, social services, or other mental health related fields.

Your Impact as a Manager of OhioRISE Care Coordination/Case Management:

Provide strategic oversight for structured service planning and care coordination, centered on high-fidelity wraparound principles.
Develop and supervise a team of Care Coordinators.
Actively engage and develop relationships with child-serving systems and behavioral health providers.
Initiate quality improvement initiatives that align with OhioRISE program requirements.
Support the development of the local system of care serving youth and families.

Qualifications of a Successful Manager of OhioRISE Care Coordination/Case Management:

Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or higher.
Demonstrated supervisory or managerial experience in community-based services.
Bring a minimum of 3 years of experience in children's mental health or related public sectors, providing community-based services to children, youth, families, or caregivers.
Display experience in managing complex cases involving multi-system youth.
Monitor timely and accurate documentation and coach staff to performance.
Maintain an outstanding driving record.
Ability to obtain NPI and Ohio Medicaid numbers (assistance available).
