Position: Autism Support Broker

Job ID: 842

Location: Westwood MA

Shift: Monday-Thursday 8:30am-4:30pm, F 8:00am-4:00pm

Salary: $53,587.50

Lifeworks is hiring a full time Autism Support Broker for the Children's Autism Waiver Program to work with families who have children with Autism. We are a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve.

Essential Duties:

Initiate and maintain contact with each family in the caseload, assessing needs and ensuring follow-through on program requirements.
Plan, coordinate and help manage the delivery of in-home behavioral services, purchase of goods and other essential services.
Maintain open lines of communication with DDS Clinical Managers. Monthly Meetings with DDS.
Develop the individual's person-centered Autism Support Plan and Emergency Fact Sheet.
Enter and maintain data in a designated consumer database.
Coordinate monthly meetings with each family on the caseload.
Assist family in hiring, scheduling and paying support workers, respite care and good & services.
Maintain necessary records documenting work performed: travel, mileage, contacts made, and a database of individuals served.
Assist the Director and staff of the ASC in the planning, development, coordination, management of services and supports to families.
Ability to work a flexible schedule to meet the needs of individuals and facilitate programs.
Excellent interpersonal skills.
Ability to multitask and prioritize tasks with attention to detail.
Ability to work in a team environment.
Daily travel in the Newton/South Norfolk & Metro-Boston area.

Requirements:

Experience working with Individuals with ID/DD
BA/BS and experience in the Human Services Field, or Associates Degree plus 5 years’ experience working in the Human Services Field.
Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
Valid Driver's License required
Excellent verbal and written communication skills
