Job Overview

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate comprehensive patient care plans, facilitate effective communication among multidisciplinary teams, and ensure seamless transitions across various levels of care. Your expertise will help optimize patient outcomes, promote efficient utilization of resources, and uphold the highest standards of health regulation policies. This position offers an exciting opportunity to make a meaningful difference in patients’ lives through proactive case management and personalized care.

Duties

Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs.
Develop and implement individualized care plans in collaboration with healthcare professionals across hospital, outpatient, and post-acute settings.
Manage clinical case management activities such as discharge planning, home health coordination, and post-acute care arrangements.
Utilize Electronic Health Records (EHR) systems to document patient interactions, medical records, and treatment progress accurately and securely in compliance with HIPAA regulations.
Facilitate diagnostic evaluations and coordinate with specialists in areas including critical care, trauma centers, ICU, emergency medicine, infectious disease care, geriatrics, behavioral health, pediatrics, and hospice care.
Oversee utilization management by reviewing medical necessity for treatments and procedures to ensure appropriate resource use while maintaining quality patient service.
Collaborate effectively with healthcare professionals to optimize patient care plans, monitor progress, and adjust interventions as needed.

Requirements

Valid nursing license with current registration; experience as a registered nurse is essential.
Proven clinical experience in hospital medicine, emergency room settings, ICU, or Level I/II trauma centers; ICU or trauma center experience highly preferred.
Strong knowledge of medical terminology, physiology, and health regulation policies such as HIPAA.
Demonstrated expertise in patient assessment, case management processes, discharge planning, and utilization management.
Experience working with Electronic Health Records (EHR) or Electronic Medical Records (EMR) systems for documentation and data management.
Excellent client communication skills with the ability to collaborate effectively with multidisciplinary teams across inpatient and outpatient environments.
Background in managing patients with complex conditions including acute care cases such as infectious diseases or post-surgical recovery; experience in geriatrics or behavioral health is a plus.
Ability to manage multiple priorities efficiently while maintaining a high standard of patient service in fast-paced settings such as hospital wards or clinics.
