Summary
We are seeking a compassionate and skilled Case Management Specialist to join our team at The Arc of Cape May County. In this role, you will support the Case Management department specifically in the provision of services and supports to individuals with intellectual or developmental disabilities for planning, organizing, coordinating, and facilitating health care related activities. This includes monitoring electronic health records, attending medical appointments, completing critical logs and other relevant documentation to ensure information is up to date and accurate. This role provides guidance and support to the agency and planning teams to ensure quality standards are consistently met and healthcare and social services needs are satisfied routinely and as needed.
Responsibilities
- Organizes and coordinates health care appointments and provides service delivery of the same as needed. Serves as primary point of contact for scheduling of annual medical and dental services including routine specialty care;
- Provides reminders and follow up communication and documentation to staff in residential and day program settings as needed for coordination of transportation and setting calendars for appointments to provide most efficient use of resources available;
- Comprehensive and detailed review of NJISPs to ensure accuracy, consistency, and accountability in coordination of health care and social services supports. Communicates with supervisor regarding updates and revisions needed to NJISPs, completes revision worksheets and supporting completion of NJISP planning activities as directed;
- Coordination and review of documentation needed, including assessments and other available records, prior to admission, transfer, re-admission of person supported and upon discharge to ensure continuity of care during their stay at The Arc;
- Reviews electronic health records, forms, correspondence, service plans, medications, and communications (T-logs);
- Assists with monitoring medication administration records (eMAR), especially as it relates to the pass along of information to the medical providers, and monthly reconciliation of medications;
- Follow-up with external agencies including health care professionals, psychiatrists, medical specialists, dental providers, physical and occupational therapy, day service programs, and other external partners as needed to ensure necessary care, follow up care, and continuity of services.
Financial
- Assist in meeting the needs of clients through the use of individuals insurance and provides receipts for any approved expenses incurred by the individual or the agency during the course of facilitating the appointment(s)
Documentation
- Maintains timely, detailed documentation to ensure adequate funding levels are met and compliance with state agencies is captured while simultaneously recording individual progress which will ensure continuity of support to clients.
Direct Support Services
- Provide personalized, one-on-one daily support to clients to attend to their physical and emotional needs in a safe and nurturing environment that promotes self-determination, respect, fairness and integrity.
Qualifications
- Must be at least 18 years old and have a High School Diploma or equivalent, Associate's Degree or 60 credit hours towards related degree; in absence of degree, must be currently actively enrolled toward completion;
- Two to three years of experience supporting individuals with developmental disabilities in residential settings preferred;
- Must be willing and able to work a flexible schedule as assigned or established to ensure needs of the individuals are met
