Full Job Description

Description: HCD International is seeking a Community Health Worker professional who can build trusting relationships between health/social services and the community we serve. Community Health Workers facilitate access to services and improve the quality and cultural competence of service delivery. Focus on ways to build individual and community capacity by increasing health knowledge and self-sufficiency. Provide information to individuals and the community we serve through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. Community Health Workers play an integral role in helping systems become more culturally appropriate and relevant to the people the systems serve.

Requirements:

High School Diploma or GED; preferred bachelor’s degree in Health Education
At least 1 year of experience directly related to the duties and responsibilities specified.
Bilingual - Spanish, English
Certified in New Mexico (Not required but a plus)
Understands Turquoise Care (New Mexico's Medicaid) and/or experience as Benefits Navigator/Enrollment (Medicaid application assistance, Medicaid renewal assistance) (Not required but a plus)

Key Responsibilities and Abilities:

Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
Knowledge of community agencies and resources.
Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.
Ability to plan, implement, and evaluate individual client care plans.
Knowledge of transportation and other barriers to care that may be encountered by the client.
Ability to communicate medical information to health care professionals and care coordinators over the telephone.
Skill in the use of personal computers and related software applications, including e-mail.
Skill in organizing resources and establishing priorities.
Creative and analytical thinking.
Help individuals, families, groups, and communities develop their capacity and access to resources, including health insurance, food, housing, quality care, and health information.
Facilitate communication and client empowerment in interactions with health care/social service systems.
Help health care and social service systems become culturally relevant and responsive to their service population.
Help to build understanding and social capital to support healthier behaviors and lifestyle choices.
Deliver health information using culturally appropriate terms and concepts.
Linking people to health care/social service resources.
Provide informal counseling, support, and follow-up.
Advocate for local health needs.
