Part time Registered Nurse Case Manager

Anticipated Territory : Greater Sacramento County

Schedule: Every Saturday and Sunday full days. Would consider Saturday/Sunday plus 3 week days.

What you’ll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

To be a success:

Current and unrestricted CA RN license
Current BLS/CPR Certification
Valid CA Driver's License
Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
High level of clinical knowledge in home health care is highly preferred
Experience with high acuity patients is a plus
Experience working with an interdisciplinary team is highly desired
Case Management fundamentals is highly desired
Previous experience with IV Medication infusions highly preferred
Proficiencies initiating intravenous (IV) therapy is a plus

Compensation: Per-Visit (PPV) – Weekend Role

This position is compensated on a per-visit basis, with rates varying by visit type.

Visit rates range from $90 to $150 per visit, depending on visit type and documentation requirements.

Expected volume: Minimum expectation of 10 visits per weekend
Estimated earnings range: up to~$1,500 per weekend, depending on visit mix
