Full Job Description

The Care Navigator provides direct, trauma-informed, recovery-oriented services to individuals from diverse communities across the Mid-Shore region. This role focuses on relationship-based case management, advocacy, skill-building, and systems navigation while maintaining clear non-clinical boundaries. Care Navigation services are time-limited and skills-focused, with an emphasis on increasing client independence, self-sufficiency, and overall well-being. This position includes both office-based and community-based work and may require travel throughout the community.

The Care Navigator works with individuals from diverse backgrounds and communities, including but not limited to Latino/a/e communities, African American communities, LGBTQ+ individuals, and first responders.

Duties and Responsibilities

Direct Client Services

Establish trusting, culturally responsive relationships with clients receiving Care Navigation services.
In collaboration with clients and clinicians, identify Health-Related Social Needs (HRSN) and other life stressors impacting mental health and overall wellness.
Assist clients and clinicians in identifying mental health or trauma-related skill deficits that impact access to resources and support systems.
Meet regularly with clients to assist with accessing community resources and support services.
Provide skill-building support related to self-regulation, symptom management, problem-solving, and self-advocacy.
Provide psychoeducation related to wellness, recovery, coping strategies, and available community resources.
Utilize risk assessment tools as appropriate and assist clients with accessing emergency resources when necessary.
Facilitate psychoeducational and skill-building groups as assigned.
Provide services in office settings and community-based locations as appropriate to client needs.

Communication and Collaboration

Develop and maintain knowledge of community resources related to housing, transportation, food insecurity, financial assistance, public benefits, and related support services.
Communicate regularly with primary clinicians and treatment teams regarding client progress and service coordination.
Participate in treatment planning and interdisciplinary collaboration as needed.
Build and maintain positive working relationships with community providers and referral partners.

Documentation and Compliance

Maintain timely, accurate, and complete documentation within the electronic health record (EHR), including contact notes, releases of information, consents, and related documentation.
Maintain confidentiality and compliance with HIPAA regulations and agency policies.
Ensure services are documented in accordance with agency standards and funding requirements.

Required Skills and Abilities

Ability to build rapport and maintain professional relationships with individuals from diverse backgrounds.
Excellent communication, organization, and documentation skills.
Ability to work independently and collaboratively within a multidisciplinary team environment.
Ability to navigate community systems and connect clients with appropriate resources and supports.
Strong problem-solving and advocacy skills.
Ability to maintain professional boundaries and confidentiality.
Ability to work effectively in office and community-based settings.
Proficiency in Microsoft Office programs and electronic health record systems preferred.
Ability to maintain flexibility with scheduling and community-based responsibilities.
Valid driver’s license and reliable transportation required.

Education and Experience

Required

Bachelor’s degree in social services, human services, psychology, social work, or a related field required.
Experience providing direct services to individuals from diverse backgrounds required.
Experience working in trauma-informed and recovery-oriented settings preferred.

Preferred

Experience working with Latino/a/x communities, LGBTQ+ individuals, first responders, and/or military-connected populations preferred.
Bilingual English/Spanish preferred.
Group facilitation experience preferred.

Work Environment

Combination of office-based and community-based work environments.
May require occasional evening or flexible hours based on client and program needs.

Pay: $54,000.00 - $58,000.00 per year
