We are seeking a dedicated and compassionate Case Manager to join our dynamic team. Responsibilities
- Conduct thorough assessments to identify clients’ needs across behavioral health, social work, child welfare, and workforce development domains
- Develop and implement individualized care plans that align with clients’ goals and circumstances
- Provide crisis intervention and crisis management support during urgent situations to ensure safety and stability
- Offer addiction counseling and behavioral health guidance to promote recovery and well-being
- Utilize motivational interviewing techniques to encourage positive behavioral changes and enhance client engagement
- Coordinate with external agencies such as Section 8 housing programs, Medicare services, and child welfare organizations to facilitate access to resources
- Supervise and mentor junior staff or interns, fostering professional growth and ensuring quality service delivery
- Maintain detailed documentation of client interactions, progress notes, and service plans in compliance with organizational policies
- Advocate for clients’ needs by connecting them with childcare services, mental health resources, or workforce development programs as appropriate
Experience
- Proven experience in case management within social work, behavioral health, or HIV related fields
- Strong background in crisis intervention and crisis management techniques
- Hands-on experience with HIV treatment,
- Familiarity with social service systems
- Demonstrated supervisory or mentoring experience with team members or interns
- Knowledge of child welfare practices and childcare considerations in case planning
- Excellent communication skills with the ability to motivate clients through motivational interviewing approaches
- Experience with Insurance Programs/Enrollment
- Experience with eCW preferred
- Ryan White experience desired
- 500/501 Certification
