Position Summary:

Agate Integrated & Behavioral Healthcare Services, Inc. is seeking a compassionate and collaborative Case Manager to join our Assisted Outpatient Treatment (AOT) Care Coordination Team. The Psychiatrist will play a vital role in coordinating services, connecting participants with community resources, monitoring each participant’s progress, and advocating for participants to ensure they receive recovery-oriented care.

This position offers the opportunity to work within a multidisciplinary team dedicated to improving treatment engagement, reducing psychiatric hospitalizations, and supporting individuals in achieving stability and independence within the community.

Essential Duties & Responsibilities:

Review participants' mental health, social, familial, cultural, medical, developmental, legal, vocational, and economic information to assist in the development of individualized AOT Treatment Plans.
Collaborate with the AOT Psychiatrist by providing comprehensive clinical and psychosocial information to support treatment planning and service recommendations.
Coordinate with the multidisciplinary AOT Care Coordination Team to develop, implement, and monitor individualized treatment plans.
Identify participant needs and connect individuals with appropriate community-based behavioral health, medical, housing, vocational, educational, and social service providers.
Make referrals for services identified within each participant's AOT Treatment Plan and monitor follow-through with providers.
Provide ongoing case management services as outlined in each participant's AOT Treatment Plan.
Maintain regular communication with participants, family members (when appropriate), treatment providers, hospitals, and community partners to ensure continuity of care.
Monitor participant progress, identify barriers to treatment engagement, and assist in resolving obstacles to successful community living.
Participate in multidisciplinary team meetings, case conferences, and treatment planning sessions.
Maintain timely, accurate, and compliant documentation in accordance with Maryland Department of Health (MDH), organizational policies, and applicable regulations.
Assist with crisis intervention, participant engagement, and care coordination as needed.
Maintain confidentiality and uphold HIPAA and all applicable federal and state regulations.
Coordinate transition planning before expiration of an AOT Order.
Develop comprehensive aftercare and continuity-of-care plans with community providers to promote long-term stability.
Facilitate warm handoffs to ongoing providers and monitor successful linkage to continuing services.
Recognize behavioral health crises and coordinate timely intervention with the Psychiatrist, Program Director, Peer Recovery Specialist, crisis response teams, hospitals, and other emergency resources when clinically indicated.
Participate in multidisciplinary case conferences, quality improvement initiatives, supervision, and staff meetings.
Work collaboratively with Certified Peer Recovery Specialists and other care coordination staff to promote participant engagement and recovery.

Minimum Qualifications:

Applicants must meet one of the following qualifications:

Five (5) years of experience providing:
Direct services to individuals living with serious and persistent mental illness; or
Mental health peer support services.

A Bachelor's degree may substitute for three (3) years of the required experience.

Preferred Qualifications:

Bachelor's degree in Social Work, Psychology, Human Services, or Counseling is required.
Licensed Master Social Worker (LMSW) in the State of Maryland, Licensed Clinical Professional Counselor (LCPC), Licensed Graduate Professional Counselor (LGPC), Rehabilitation Counselor.
Experience providing case management or care coordination within behavioral health, community mental health, Assertive Community Treatment (ACT), psychiatric rehabilitation, crisis services, or similar settings.
Knowledge of serious mental illness, recovery-oriented practices, trauma-informed care, motivational interviewing, and community-based behavioral health resources.
Strong organizational, communication, and interdisciplinary collaboration skills.
Experience working with multidisciplinary teams and community partners.
Knowledge of Maryland public benefits systems (Medicaid, SSI/SSDI, HealthChoice) strongly preferred.
Experience working with individuals with serious mental illness (SMI) in community-based settings preferred

Proficiency in electronic health record (EHR) systems required.
