Full Job Description

We are looking for all service areas: Brooklyn, Queens, Manhattan, the Bronx, and Nassau county. This is a Fee for Service role (Per diem) for Weekend hospice admissions.

JOB SUMMARY: Provides nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary Plan of Care. Supervises Home Health Aides. Supports the Provider Relations and Quality Improvement activities of the organization.

RESPONSIBILITIES:

Receives patient assignments from the After Hours Nurse Manager.
Assumes responsibility for a patient/family that includes assessing, planning, implementing and evaluating care within the first 24 hours of patient/family admission.
Ensures that the plan of care reflects the patient’s current conditions and needs based upon the initial and all subsequent assessments.
Nursing visit notes address and reflect those issues that are on the plan of care. New conditions or changes are reflected on the Plan of care through the hospice IDT plan of care update. Assumes leadership in all modifications of care planning and ensures all interventions are implemented as planned/prescribed.
Initiates communication with attending physicians, other hospice staff members, contract providers, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.

Nursing Home Facility:

Promotes and maintains a collaborative relationship with facility staff in the delivery of an integrated plan of care within a skilled nursing facility.

Inpatient Care Facility:

Ensures that there is a q-24-hour clinical note written by facility staff or hospice staff justifying the patient needing a continuation on this level of care. Absence of such notation will be brought to the attention of the unit supervisor and to the hospice nurse coordinator & or Q.I. RN.
Ensures that all facility hospice records are kept up to date with care plan updates, assessments and other medical records/documentation concerning the hospice patient/guardian.
Respond to new Referrals in the absence of a Hospice Liaison.
If there are no clinical assignments, circulate in the IPU to visit new admissions as directed.
Maintains regular communication with the Hospice Nurse Coordinator to review care.
Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the Plan of Care.
Maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
Accepts responsibility for coordinating the physical care of the patient by teaching primary caregivers, volunteers, employed caregivers, and/or by providing direct care as appropriate.
Instructs the patient and caregiver:
How to administer medication and recognize side effects.
How to perform personal care and oral hygiene.
How to assist in lifting, moving, ambulating and exercise.
How to prepare for the events preceding death.
Informs the Patient Care Coordinator of unusual or potentially problematic patient/family issues.
Provides appropriate support at time of death.
Participates in hospice orientation and ongoing education programs.
Demonstrates familiarity with policies of the agency and state(s) and federal regulations pertaining to nursing services.

QUALIFICATIONS:

Registered nurse currently licensed in the state.
Baccalaureate Degree in nursing from an approved program (BSN); or
Registered professional nurse with two years’ experience in either home health care or Hospice nursing.
Knowledge of Computer operation, computer generated clinical records experience preferred.
Demonstrates commitment to Hospice philosophy of care.
A reliable automobile, valid driver’s license, and the minimum state required liability automobile insurance is required.
